Infrequent Vendors - Visiting 3 times a week or less. We will add these vendors to the visitor's logbook at the front gate.
Fax or email temporary vendor memo from the Dept. Head/Rep - to include vendor's:
Name, Company, SSN, DOB, POC and telephone number to:

Frequent Vendors - Visiting more than twice a week. We will also add these vendor's to the visitor's logbook at the front gate. They may also come to pass & decal office for a one-year hospital badge and three-month vehicle pass (to be renewed every 90 days).
Fax or email vendor memo from the Service Line Leader - to include:
Name, Company, SSN, DOB, POC and telephone number.
Also, include a privacy act statement (NMCP 5512/8 (rev 4/07) for each vendor to:
Pass & Decal 953-6951 or email.
If you know or can estimate how often and how long the vendor will be on base, please add to the memo.
