 Office of Vital Statistics

Birth/Death Certificates

This Office is the legal custodian of birth and death certificates filed in the state of Nevada from July 1, 1911 to the present.  For events prior to 1911, records are usually held in the county Recorder’s office where the event occurred.  A list of the county Recorder’s offices can be found on our application for a marriage or divorce record.  Birth and death records are confidential in the State of Nevada and may only be released to a qualified applicant. A qualified applicant is defined as the registrant, or a direct family member by blood or marriage, his or her guardian, or his or her legal representative.

  • To order online through vitalchek:

Certified copies of birth and death records may also be requested via the Internet. Please contact www.vitalchek.com for instructions.

  • To order by mail:

Office of Vital Records

4150 Technology Way, Suite 104

Carson City, Nevada 89706

Telephone: 775-684-4242

Fax: 775-684-4156

If you are a qualified applicant, you can complete an application form and mail it in with a check or money order for the appropriate fees.  Certified copies of birth records are $13.00 each; certified copies of death records are $10.00 each.    For questions on how to amend or correct information on a birth or death record, please contact our office for instructions.

Effective June 2, 2005, the Nevada Legislature passed Senate Bill 118 (SB118) which authorizes each county in Nevada to create a bank account for the collection of fees to support their County Coroner's office.  Per SB118, as each county authorizes the creation of the account, the fee charged for a certified copy of a death certificate issued for any person who dies in that county will increase by $1.00.  This means that the fee charged for certified copies of death certificates in some counties will be $11.00; in other counties the fee will remain $10.00.

As of April 10, 2006, only Clark, Douglas, Mineral and Washoe counties have notified this office of the creation of their coroner's support accounts.  If the person whose death certificate you are requesting died in one of these four counties, please send $11.00 for each copy desired.  If you have any questions regarding this increase, or for the most current fee information, please contact this office at (775)684-4242.

Marriage and Divorce Records

Please note that marriage and divorce records are held at the county level.  For certified copies of marriage certificates, you must write to the County Recorder in the county where the license was purchased.  For divorce records, you must write to the County Clerk in the county where the divorce was granted.  Our Office can search for and verify events occurring after 1968 if you submit a request containing the full name(s) of the person whose record you are searching for, the approximate date of the event and the city where you believe the event occurred.  The fee for verification of a record is $8.00 for each name searched.

For any questions regarding Vital Records, please contact our office at 775-684-4242, or e-mail us at vitalrecords@health.nv.gov
