Oregon Family Planning Manual CoverOregon Family Planning Program Manual

The Oregon Family Planning Program Manual is divided into six sections. Clinics may need some or all of them, depending on whether they are FPEP-only or FPEP and Title X, and if they participate in the IPP project. All clinics should download:

          Cover / Introduction / Table of Contents

          Section A / Overview

          Section D / CVR Manual

          Section F / Appendices

plus their program-specific manual. Individual pages and sections will be updated periodically, and will be available for download at that time.

      2008 FPEP Trainings 

 

      

Register now for August FPEP Trainings in Portland! All participants must complete a Training Registration Form (Word) (pdf ) by July 14, 2008.

 

Videoconferences must be organized by local staff. A Videoconference Host must complete the VCON Registration Form (Word) (pdf) by July 14, 2008.

  • FPEP Orientation:  Introduction to the Family Planning Expansion Project
    • Monday, August 11th  12:30 - 4:00   (Portland, VCON, Register by 7/14/08)
    • Tuesday, October 28th  1:00 - 4:30 p.m.   (NETLINK, Register by 10/22/08)
  • Program Integrity Plan:  What you need to know to keep the dough
    • Tuesday, August 12th  9:00 - 11:30 a.m.   (Portland, VCON, Register by 7/14/08)
    • Tuesday, November 18th  1:00 - 4:30 p.m.   (Register by 11/12/08)

     How to Become an FPEP Provider

 

  1. To start the enrollment process, read through the Provider Enrollment Memo (PDF) and the Prospective Provider Packet (PDF) to determine if your clinic can comply with FPEP program requirements.  If so, then download and complete these forms:

  2. Mail or deliver the completed Application and Project/Site Form to the address on the Application.

  3. Next, download and read through the FPEP Manual  and FPEP Administrative Rules (PDF)
