Certification applications are now available in both word-fillable and pdf formats. You may choose a "word-fillable format" to complete on your computer and then print out or a "pdf" to print out and then complete.
For DBE firms only. Firms applying for recertification in the Disadvantaged Business Enterprise program must also submit a current Personal Financial Statement document. Personal Financial Statement.doc "fillable" Personal Financial Statement.pdf
Certification applications can only be accepted by mail or hand delivery.
We cannot accept faxed certification applications.
