Amusement Park and Carnival Ride Operators must:
Obtain registration application. A renewal permit must be filed annually.
Obtain permit for entire facility ($100/yr.)
Prove the following coverage: (a) $1million per person for bodily injury in one person accident, (b) $500,000 for bodily injury to more than one person, and (c) $10,000 for destruction of property
Evidence of safety and current proof that rides are safe.
Letter from financial institution showing good standing.
Proof that any changes that have to be made have been made.

Licenses ski lifts, sky rides, incline elevators, tramways, ski tows, aerial rides, etc. that are attached to or propelled by cable. licenses are issued in conjunction with the annual licensing inspection.
License and fee varies by type of lift
Additional forms must be submitted for the installation of New Tramways and the Relocation of existing tramways.

NOTE:  Check with local licensing authorities for local regulation of permanent or temporary public facilities used for performances, amusement devices, etc.
NOTE:  If facility has a water slide, swimming pool, or other public or semi-public water facility (including hot tubs and saunas), see SWIMMING POOL
