The early registration fee will be $315 (US dollars) payable by check, Visa, or Master Card and due by April 15, 2003. All funds are payable in US dollars. Simply fill out the form below to let us know you will be attending or, if you wish to pre-register with your Credit Card, please use our secure registration form instead.

Late registration will be available at the conference for $350 (U.S. dollars) payable by check, Visa, or Master Card.

The registration fee covers attendance of the conference, a reception Monday night, lunch on Tuesday, Wednesday, and Thursday, the dinner reception on Wednesday night, and an on-site copy of the conference proceedings. Hotel accommodations must be booked separately at the Park Vista Hotel. Please reference the QCAV 2003 conference to receive the conference hotel rate.
