You must provide the following information.
(1) Identify all entities directly involved in the ownership and control of the facility, including electric utilities and non-utility entities.
(2) Show all ownership and control percentages.
(3) For all non-utility entities, identify upstream entities which own or control 10% or more of the non-electric utility entity.
(4) Identify any other subsidiary of a non-electric entity that owns or controls 10% or more of the non-utility entity.
