Aspectos generales de las opciones de tratamiento
La terapia mínima para las malignidades de grado bajo de la porción superficial
de la glándula parótida consiste en una parotidectomía superficial. Para toda
otra lesión, a menudo se indica una parotidectomía total. Debe hacerse resección
del nervio facial o de sus ramas si están afectados por el tumor; se puede
efectuar reparación en forma simultánea. Hay pruebas crecientes que indican que la radioterapia
postoperatoria aumenta la resección quirúrgica, en particular para las neoplasias
de grado alto, cuando los márgenes están cerca o comprometidos, cuando los tumores son grandes o cuando hay prueba histológica de que hay metástasis en el ganglio linfático.[1-8] Se han
finalizado ensayos clínicos en los Estados Unidos e Inglaterra que indican que la
radiación con haz de neutrones rápidos aumenta la supervivencia libre de
enfermedad y la supervivencia general de los pacientes con tumores que no se
pueden resecar o de pacientes con neoplasias recidivantes.[9-12] Hay pocas
instalaciones médicas disponibles en los Estados Unidos en donde se realice la
radioterapia con haz de neutrones rápidos. La radioterapia con haz de fotones
hiperfraccionada acelerada también ha producido tasas elevadas de control
regional local a largo plazo.[13,14] El uso de quimioterapia para tumores malignos
de la glándula salival continua en evaluación clínica.[15-19]
