What is Abilities, Inc.?
Abilities, Inc./Edwin R. Martin Jr. Career and Employment Institute (CEI) provides
comprehensive services to help individuals with disabilities reach their employment
goals. CEI provides employment evaluation, training and placement services to more
than 1,500 adults with disabilities each year. CEI serves persons with a wide range
of disabilities including learning, developmental, hearing and visual impairments,
physical and emotional disabilities, traumatic brain injuries and health
impairments. CEI's training programs, which are approved by the New York State
Department and are directed by business representatives include: business skills, PC
applications, information technology, laboratory assistant training program, and steps
toward employment program (retailing, assembly and clerical).

Services?
Abilities, Inc. at the National Center for Disability Services, is a non-profit agency
dedicated to facilitating integrated employment and full participation of persons with
disabilities in their work and living communities. Services are provided directly to
youth and adults with disabilities, as well as to employers, insurance carriers, schools,
colleges and other rehabilitation providers.
Services to adults include vocational assessments; training in business skills,
Microsoft office specialist, laboratory assistant, retailing and office support,
remediation in English and Math, and GED preparation; and employment services such as
developing resumes and cover letters, completing applications, interview skills job
placement assistance and follow-up as well as job coaching.

Access Resources is a workplace mediation and training firm, specializing in the

What is America Works?
America Works is an employment placement service for people receiving all forms of
public assistance. We briefly train participants on the expectations and challenges
of work. We then place participants in paying jobs that match their skills and
interests. The average wage for our jobs is $8.50/hour. We work with over 500
companies. After placement, we provide intensive case management, visiting the
participants on the job weekly and coaching them on the professional and personal problems
in their way. Case managers work with small case loads of 20 participants at a time
and provide support for a minimum of six months and indefinitely as needed. We also
refer participants out to receive social services, coordinating services with the job.

What is ANIBIC?
VOICE, the Vocational Program of ANIBIC consists of Pre-Vocational and Supported Work
Services. The services are provided to assist individuals to develop the skills
required to obtain and maintain employment in the competitive work force. The
Pre-Vocational Assessment phase of the Vocational Program consists of job readiness,
vocational counseling, evaluation, and assessment at a volunteer site. Vocational
Counseling and evaluation will begin upon entry to the program to assist individuals in
overcoming barriers to work and determine interest, and ability, and skills relevant to
their vocational placement. The Vocational Counselor will work with the consumer in making
recommendations as to appropriate sites based on information obtained through evaluations,
the vocational plan and the expressed interest of the consumer. Once determined to
be ready for placement, a referral is made to Supported Work. Job Readiness
training focuses on: Daily Living Skills, Career and Interest Exploration, Job
Preparation, Self-awareness, Interview Skills, Work Assessment, Peer Support, and
Communication Skills. The Program is conducted Monday through Friday, 9:15 AM-3:20
PM. ANIBIC's Supported Work program assists individuals to obtain paid employment
through selective job placement. Services include job placement, intensive on-site
training, counseling, and advocacy. The Supported Work program also consists of a
Job Club one day per week to assist individuals in developing their own job seeking
skills.
The following are the minimum requirements for joining the VOICE program:
- members must be at least 17 years of age.
- Members must be able to travel by public transportation or capable of learning to travel
with training.
- Members must posses a true desire to participate in the Vocational Program and to enter
the work force.
- Members must also demonstrate a basic understanding of the working world and the
responsibilities it holds.

What is APNI?
APNI is a statewide nonprofit Parent Training and Information Center, serving parents
of children with disabilities from birth to adulthood since1977. The following
services are offered to parents, students and professionals in public and private schools
in Puerto Rico; individual information assistance, referrals, educational material for
anyone who contacts the office or attends any of the workshops, a bi-annual newsletter and
workshops and training sessions.
APNI has been providing high quality training to parents and professionals
in Puerto Rico for over twenty years. Project staff have developed collaborative

What is Aspire?
Aspire has been serving individuals with a variety
of disabilities for over 52 years. Aspire provides a wide range of services including
employment, residential, therapy and medical, counseling, education and early
intervention, social and recreational, day programs and service coordination. Specialized
Support Services (SSS) is the vocational and supported employment component of
Aspire.
SSS works with individuals help identify and reach their vocational goals. Many
people with diverse skills and challenges have found jobs in a wide variety of fields,
from office and computer jobs, industrial and maintenance, to retail and hospitality.
Services are highly individualized and include person-centered pre-employment exploration,
job development, job coaching and follow-along services. Individualized
non-traditional vocational assessments identify individual interests, requirements and the
contributions they can bring to an employer. SSS also oversees the Community
Employment Office, as Aspire is lead agency for this collaborative project of 27
agencies that work together to increase employment opportunities for individuals with
disabilities in the community.

What is Atlanticare Behavioral Health Human Resource Advantage?
Human Resource Advantage is a supported employment program providing community-based
vocational services to persons with barriers to employment. We do this by
innovatively assisting them in solving problems that have traditionally stood in the way
of their choosing, securing, and retaining integrated, competitive employment. These
services include individual and group employment counseling, job development, marketing to
area employers, setting up individual interviews, job coaching (if needed), and follow

What is the Bacharach Institute for Rehabilitation?
Bacharach Institute for Rehabilitation offers a number of comprehensive, inpatient and
outpatient rehabilitation programs and services designed for people with physical
disabilities. We provide organized, integrated rehabilitation programs that draw
upon the expertise of many medical disciplines. This interdisciplinary team approach to
treatment takes the patient's total functioning into account and ensures that careful,
through rehab regimens are tailored to meet each patients individual needs. As the
only state-licensed comprehensive rehab hospital in Southwestern New Jersey, Bacharach's
highly skilled treatment teams include, but is not limited to licensed physical
therapists, registered occupational therapists, Doctorate and Master's level
psychologists, Licensed Master's level audiologists, licensed Master's level vocational
rehabilitation counselor, licensed Master's level speech pathologists, certified
therapeutic recreation specialists, and certified prosthetists and orthotists.

What is the Barrier Free Living?
Barrier Free Living is a not-for-profit organization dedicated to helping people with
disabilities live independently in the community. BFL provides individuals with a
comprehensive range of transitional shelter and counseling services that enables them to
overcome the physical and emotional barriers that stand in the way of them living
dignified, secure lives.

What is the Boulevard Clubhouse?
Boulevard Clubhouse provides a welcoming, safe environment for the mentally ill. They
become known as "members" and are treated with respect. Their participation in
the functioning of the Clubhouse enhances the sentiment that they are needed and
appreciated here. We seek to provide members with experiences that mirror the community
and prepare them successfully to meet these challenges when they are further integrated
with the community at large. The stuff is multiracial and committed to the population we
serve.
Traditionally, the day starts with both members and staff sharing breakfast. Members
and staff are then separated into Four units: Maintenance, Food Service, Administrative
and Member Services. The Work Order Day is programmed to parallel the normal working
hours, this is essential to give members who are interested in independent employment in
the future the stamina and confidence to work a full day. The Four Different Units engage
members with a feeling of purpose and confidence.
All work in The Club is designed to help members regain their self-worth. The
opportunity to train on Computers, Administration and/Maintenance, or Kitchen Services
Unit is open to all. Clubhouse life is designed to strengthen and challenge each
individual to reach their potential. Upon regaining their confidence members can return to
the work force by obtaining transitional or supported employment through the Clubhouse
Vocational Services.

Presently serving more than 15,000 individuals annually, the Brooklyn Bureau is one of

What is Brooklyn Bureau of Community Service?
The Brooklyn Bureau of Community Service is one of the borough's oldest and largest not
for profit social service agency, providing a broad range of programs to children,
families and adults throughout the borough of Brooklyn.
The Adult Rehabilitation Services Department provides vocational evaluation, skills
training and placement services to individuals with disabilities. The vocational aptitude
of each participant is thoroughly evaluated. Participants then enroll in one of the
vocational training programs. The Bureau provides training in the following vocational
areas: retail store, food service, porter maintenance, assembly packaging, messenger
service, clerical support, and shipping and receiving. Counseling and support services are
simultaneously provided. Through training, participants gain marketable skills, learn
appropriate work place behaviors, and develop the abilities needed to secure and maintain
mainstream employment. Bureau Job Developers arrange for graduates to fill available
positions of employment in the private sector. Transitional job coaching and supported
employment services are available.
The Bureau offers pre-vocational services geared to community integration. Project
Moving On, a continuing day program, provides participants comprehensive mental health
services while gaining independent living skills in a supported environment. At Metro club
and East New York Clubhouses, participants develop socialization, daily living and basic
vocational skills. Other community based programs available to the developmentally
disabled include day habilitation and residential habilitation. Service coordination is an
integral part of these services.
The Bureau offers transitional housing to homeless women with mental illness.

The Brooklyn Center for Independence of the Disabled, Inc. (BCID) is a non-profit grass
roots organization represented by a majority of people with disabilities. We exist to
improve the quality of the life of Brooklyn residents with disabilities through programs
that empower them to gain greater control of their lives and achieve full and equal
integration into society. We accomplish this through our services, our advocacy for
systems change to remove physical, attitudinal and communication barriers to people with
disabilities and through our education and awareness programs. Based on a self-help
model, our primary services include: disability rights advocacy; peer support; resource
information and referral; housing advocacy; independent living skills consultation;
government benefits information and advocacy; as well as information on the civil rights
and responsibilities of people with disabilities. Many of our staff members, board
members and volunteers have disabilities themselves and so are viable role models for
people with disabilities. As a member of the National Council on Independent Living
and the Association of Independent Living Centers in New York, BCID is in a position to
advocate on many levels for people with disabilities. Our position in the disability

What is Brooklyn Community Housing and Services (BCHS)?
BCHC is a non-sectarian, non-profit agency providing housing and supportive services to
individuals and families. Its mission is to help homeless and/or mentally ill people
achieve self-reliance and independence. BCHS, originally known as Pastoral and
Educational Services, was founded in 1979 in response to President Jimmy Carter's urgent
call for community based solutions to the de-institutionalization and pending homelessness
of mentally ill individuals.
From a beginning of 24 scatter-site apartment beds, the agency developed a unique special
needs housing complex in 1991 known as Brooklyn Gardens. It provides temporary
housing of six months for single homeless, chronically psychiatrically disabled men and
women; and permanent SRO rooms and services for 74 formerly homeless adults. As the
scatter-site apartment program continued to grow, BCHS developed the "Rebuilding Our
Workforce" program in 1995 funded by HUD to assist formerly homeless individuals to
prepare for employment. In 1997 BCHS launched a new housing and employment project
funded by HUD and known as "Bridge to Home" for homeless adults with histories
of substance abuse leaving the shelter. The agency is continuing to broaden the
scope of its vocational rehabilitation and employment services available to all its
consumers through the support of its Working Community Program.

What is the Brooklyn Peer Advocacy Project?
The Brooklyn Peer Advocacy Project is the first borough-wide self-help and empowerment

What is the Business Outreach Center Network?
The Business Outreach Center Network (BOC Network) is a multi-community program
dedicated to helping isolated entrepreneurs start and grow small businesses.
Disability Jam was made possible through funding from the NYS Council on the Arts.
Disability Jam is a radio magazine series presenting political and cultural issues of
concern to the disability communities and wider listening audience. The program
examines the social, political, economic and educational controversies evoked by events
both inside and outside the disability movements. Disability Jam networks with

Step by Step is located in Fulton NY and provides a number of services to adults that
are recovering from mental illness. SBS is a voluntary member run program that serves
mental health consumers that reside in Oswego county. SBS has grown from a handful of
member beginning in 1994 to an enrollment of 150 members 2001.
The clubhouse follows the ICCD (International Center for Clubhouse Development)
standards and become a certified clubhouse in 2001. The standards require SBS to have 25%
of the average daily attendance in a transitional employment placement. TE offers our
members the opportunity to try work in a supportive setting. This means the members are
trained by SBS staff or other members to work clubhouse placement. These TE
placements are 6 to 9 months long and provide the employers with 100% coverage. If a
member can not work for any reason, a staff member works the placement free of cost to the
employer.
The clubhouse is based on a work order day where members punch in and close what unit
they would like to work in at the clubhouse. SBS has several work units to choose from and
all members and staff work side by side to keep the clubhouse running.
Work Units:
- The Kitchen/cafe serves daily low cost breakfast and lunches. This unit makes monthly
menus, grocery shops, caters all clubhouse parties and helps members with dietary skills.
- Membership outreach keeps the clubhouse statistics, provides outreach call., cards and
visits to members that may need additional support. This unit also provides tours and
orientation to new members.
- Transitional Employment & education unit provides services explained above as well
as assistance and support with educational goals. SBS hold employment dinners twice per
month to celebrate and give support to those who work during the day and can not attend
clubhouse. Employment dinners are a good opportunity for speakers to address the
membership on a variety of topics such as how work effects funding, job search, resume
writing and more.
- Peer support and advocacy provides support to members in a social capacity with
activities that are scheduled at monthly planning meetings by the members. Peer advocacy
assist other with their personal goals and any other situation that needs support. Peer
advocates look out for the legal and civil rights of mental health counselors.
- Program Office handles the administration of the clubhouse, grant writing, evaluation of
staff, and teaches office skills such as filing, faxing and letter writing. This unit also
produces the Stepping Stone newsletter.

The Catskill Center for Independence, one of New York's 35 independent
living centers is a community based advocacy agency serving the community as

What is the Center for Career Freedom?
The Center was founded in 1998 for the purpose of facilitating the recovery and
rehabilitation of persons with severe and persistent mental illness. All our staff are
survivors and have extensive experience in business, as well as counseling and social
work. This unique blend of skills provides our members with a caring and street smart
approach to reaching their goals. We see our role as a partner, to assist our members in
their recovery at their request and at their pace. With thirteen staff and volunteers, the
Center assists over three-hundred fifty psychiatric disabled men and women from 21 to 75
years, with a variety of services.
Programs range from discharge planning at psychiatric hospitals, a Drop-In Center
offering arts and crafts, music, meals, clothing, a food pantry, discussion groups, web
surfing, desktop publishing, newsletters, cards, etc., a Certified MS/Office User Training
Program: Word, Excel, Access, PowerPoint, and Outlook, vocational counseling and job
development leading to competitive employment.
Unique features: 100% off our four Board and staff are survivors. Most staff have
graduate degrees and middle management corporate (NYSE, Bell South, Pepsi Cola,
Transamerica, etc.) or small business experience. The Center offers a computer as
performance incentive to every member who achieves MS certification and is able to hold
down a job for three months.

The Center for Independence of the Disabled in New York, Inc. (CIDNY) is a not for
profit, non-residential independent living center located in Manhattan. CIDNY began
operation in 1979 and was first of thirty-four independent living centers in New York
State.
CIDNY adheres to the philosophy of the independent living movement, which is that
people with disabilities are best able to help other disabled people. The majority of
staff and the board of directors are themselves people with disabilities.
CIDNY's purpose is to help people with disabilities gain the skills and obtain the
services they need to live independently in the community. The center is also
legislatively mandated to affect changes within the community that improve the quality of
life for people with disabilities.
The center provides services to individuals with a range of disabilities. Consumers
requiring services can call the center for an intake.
Personal Assistance Care to eligible individuals with disabilities is provided by
Living Services, a separate non-profit affiliated with CIDNY.

The Center for Family Support (CFS) is a not-for-profit human service agency
established in 1954. We provide a variety of in-home and residential services to
individuals residing throughout the five boroughs of New York City, Nassau and Suffolk
Counties, Westchester and New Jersey. Administrative supervision is based out of

What is Cerebral Palsy Associations of New York State?
Cerebral Palsy Associations of New York State (CP of NYS) was founded in
1946 by parents of children with cerebral palsy, who sought critically

What is the Columbia University Center for Social Policy and Practice
in the Workplace?
The Center for Social Policy and Practice in the Workplace (Workplace Center). a
research, program development, consultation and training institute operating as an
integral part of Columbia University, was established in 1969. Its mission is to
study conditions at the workplace and in the community that place populations at risk of
losing their employment, or of being able to secure employment, and to develop program
responses to its findings. Funded by public and private sources, the Workplace
Center has a history of helping corporations, trade unions and social agencies and
facilities deal with workforce issues including disability, gender integration, domestic
violence, diversity, and substance abuse, among others. Recent Center projects
include several studies funded by the National Institute of Disability and Rehabilitation
Research (NIDRR) to identify the workplace consequences of disability, and a pilot study
funded by New York Community Trust to explore the effectiveness of an intervention
developed from an NIDRR project. The Center director is Dr. Sheila H. Akabas, an
internationally renowned expert, scholar and author on social issues in the workplace.

Community Access Unlimited is a private, not-for-profit social service agency that has

What is Community Options, Inc.?
Community Options of New York City provides services to individuals with disabilities
to live independently in their own home and secure employment in the community. The office
supports people living in all the five boroughs of New York City and Westchester County.

The Public Benefits Resource Center (PBRC) of the Community Service Society of New York

Who Are We?
Compeer is a not-for-profit companionship program that is based on the concept that a
volunteer's friendship and support can offset the loneliness and isolation that accompany
mental illness. the help that persons coping with emotional problems need is often more
than traditional therapy can offer. They need the support of a friend. They need a Compeer
volunteer.
What Does Compeer Do?
Compeer recruits, screens, and matches trained volunteers in one-on-one friendship
relationships with adults receiving mental health treatment. Volunteers become positive
role models, help raise self-esteem, increase social and communication skills, and
encourage independence for their Compeer friends.

Our Local Northwest Area, was designated by our Governor, to administrate the Federal
Funds of the Workforce Investment Act of August 7, 1998, "WIA". To serve the
municipalities of: Aguada, Aguadilla, Anasco, Isabela, Moca, Rincon, and San Sebastion.
The activities and service under the Workforce Investment Act "WIA", are
directed to serve young people, adults, and employed that need these services to retain
their jobs. Also, unemployed and displaced workers. It offers young people and adults a
range of activities that enable them a suitable way to face the challenges of the labor
market, providing them necessary information and orientation to reach occupational goals.

What is the Contemporary Guidance Services, Inc.?
Contemporary Guidance Services, Inc. (CGS) is a non-profit agency with two centers in
New York City (Manhattan). Since 1974, we've helped launch developmentally disabled
adults on great careers in such fields as retail, mailroom/messenger, food service,
technology, nursing assistant/home health aide hospitality (hotel jobs.) We also
offer vocational counseling, psychotherapy, speech/communication therapy, academic skills
classes, occupational therapy and social work services - all in one place! We start
everyone out with a 15 day evaluation to find out what they do best; then we recommend
either a 195-day training program, our pre-vocational class, supported employment or our
Clerical Work Activity Center (sheltered employment). Our training covers everything they
need to succeed in any job; dress and grooming, getting along with supervisors and
co-workers, workplace behavior and what to expect at work. Once they're in a
training program, we send them out on internships where they use what they've learned in
our classes and get firsthand experience in their future careers. And very often, those
internships become paid jobs once the student graduates from CGS! Placement and lifetime
follow-up services await our graduates. And it's all free! Our graduates are
now working in law firms, major corporations, hospitals, department stores, fine hotels,
restaurants and health clubs. We're always updating to insure that CGS graduates are
poised to take full advantage of the 21st century job market!

What is the Corporation for Supportive Housing?
The Corporation for Supportive Housing is the first national intermediary dedicated to
strengthening the supportive housing industry. CSH provides grants, loans and
technical assistance to nonprofit developers, facilitates national innovation exchange,
leverages substantial corporate financing and works with every level of government to
coordinate the process of building more and better supportive housing so that America can
put an end to homelessness for its most disenfranchised citizens.
CSH's mission is to build a network of locally based nonprofit housing and service
providers competent and qualified to develop and operate supportive housing on a scale
that will have a discernible impact on the need.
Employment
Once people are stable and housed, their first priority is to find work. But tenants
face myriad barriers including lack of training, lack of access, lack of supports, and
lack of appropriate jobs. Increasingly, supportive housing providers, assisted by
CSH's Next Step: Jobs initiative, are helping tenants reconnect to the working world.
These organizations provide education, job training and coaching, interview preparation,
in-house employment, jobs in other non-profit businesses and linkages to mainstream
employment.

What is DebRA Inc.?
DebRA of America is a not-for-profit organization that provides advocacy and support
for those persons affected by EB and their respective families. DebRA stands for the
Dystrophic Epidermolysis Bullosa Research Association.
Epidermolysis Bullosa is a rare genetic skin disease that cause blisters and fragility
of skin and mucus membranes. There are three main types of EB with subtypes that vary in
degrees of severity .
People with Epidermolysis Bullosa can suffer with mild to severe discomfort,
disfigurement and many times death.
In addition, DebRA of America promotes and supports research for EB, assists in
locating social, medical and genetic counseling. Provides educational material to health

Special Child, Adult and Early Intervention is a ate program that is
part of the Department of Health and Senior Services. The program's mission is to ensure

materials and written exercises. Placement staff discusses all facets of the interview
process and offers employment counseling. We provide well-crafted resumes and cover
letters focused on vocational goals.
EEC provides enlargers, in-house interpreters, specialized computer equipment,
dedicated telephone lines, bilingual staff and others modes of communication to assist the
job seeker.
All consumers are provided post-employment follow-up in review of their achievement and
satisfaction, along with our policy of long-term assistance.

What is Cornell University/ Program on Employment and Disability?
Program on Employment and Disability (PED) staff members provide technical assistance,
operate as consultants, and also offer standardized courses and print materials.
They arrange workshops, seminars and large conferences for more than five hundred
participants at a time, and work on general or specific issues with individual clients.
A cluster of PED courses -- on topics such as supported employment, management of
centers for independent living, or the work incentive provisions of the Social Security
Act -- can lead to a valued professional credential, a certificate of completion from
Cornell University. PED's series of forty simple and straightforward guides and
training packets on the American with Disabilities Act has earned praise from human
resource managers and employment opportunity advocates alike.
PED specializes in marshaling resources -- and breaking down barriers -- within
organizations. With companies, that often means bringing together managers and
representatives from human resources, benefits planning, facilities, and risk
management. In a municipality, it may involve cooperative work among such
departments as Human Resources, Recreation, Public Works, and Facilities Management.
In an education setting, PED focuses on enhancing workplace preparation for students with

Employment Consultants assist both employers and people living with disabilities in
developing good working relationships. The applicant and consultant search to find

designed to insure long-term successful employment.
Since 1948, Enable's mission has been to enable people with disabilities to participate
fully in all aspects of life. We provide assessment, training, therapy and support for

provides information, referral and services to children and adults with cerebral palsy and
other developmental disabilities through medical and diagnostic clinic; educational

What is the ESRD Network of New York?
ESRD of New York is a voluntary not-for-profit organization dedicated to improving the
standard of care for renal patients. It serves dialysis facilities and kidney
transplant centers that provide treatment to over 20,000 ESRD patients in New York
State. It is part of the national system of ESRD Networks established by Congress to
help facilities achieve high standards of care for renal patients. The Network
Rehabilitation Committee works to increase employment and education opportunities for ESRD
patients by providing information to state and local agencies regarding employment of ESRD
patients and by informing renal care facilities of rehabilitation opportunities for their
patients.

What is the Fair Lawn Deaf Program?
The Fair Lawn Deaf Program opened it's doors in 1977. It is a vocational service
agency, funded by the Department of Labor to assist the Deaf, Hard of Hearing and Late
Deafened residents of New Jersey in every aspect of their employment pursuits. The staff
is fluent in Sign Language and comfortable with the range communication styles as well as
the academic and vocational levels involved in serving deaf adult.
Approximately 200 deaf,
hard of hearing and late deafened adults are seen annually. Clients who do not have
training, former work history, or want to change fields are evaluated through work samples
and psychometric tests. These tests, which do not rely on auditory cues, help to determine
a client's interests skills and abilities so that realistic career goals can be developed.
Results are reviewed and discussed at a conference, which is attended by staff, client
referral source and/ or any other relevant people. the conference may, but does not
always, result in job search. Some clients are students, and are evaluated to help in
curriculum and post-secondary planning. Other clients may not yet be ready for work, and
could in stead be recommended for training programs or workshops. Health related issues
might prevent other clients from pursuing a job.
Work Adjustment Training and job counseling are provided for clients
who will be involved in a job search. They are helped by practicing with employment
applications, reading classified ads, and by role-playing interviews with an interpreter.
Counseling is provided to clarify goals, determine salary guidelines, transportation
limitations, and to help the person prepare for employment. Some clients are not assessed
because they have been evaluated previously and are seeking similar employment again.

who have a marketable skill, who are laid off or want to change companies, but want to stay
in their field. Once references are verified, interview practice is provided, and search
is started.
A job search begins by using a file of employees who have shown an
interest in utilizing the program's services. Many companies have hired deaf individuals
who have been successful employees, and are then interested in hiring additional qualified
people. Suitable employees are contacted from the roster, "help wanted" ads are
used and also companies are "cold canvassed" for the purpose of obtaining a job
interview. All efforts are made to match an applicant and job in as many areas as
possible.
Interpreter services are provided to all clients and employees who want
an interpreter, for job interviews and for the initial stages of job placement, to ensure
full communication between the employer/supervisor and the potential employee. In
addition, interpreter services are provided for on-the-job training, meetings, or
troubleshooting to help deaf employees retain jobs, whenever possible. Employees know that
they can call upon the Fair Lawn Deal Program at any time.
The people placed range from unskilled workers (factory, warehouse) to
skilled workers (clerical, machinists) to technicians and professionals (programmers,
accountants) in a wide variety of occupations. All clients have experienced some degree of
hearing loss. Many use Sign Language as their modality of communication, and to varied
degrees use speech reading as well as residual hearing to communicate. Communication style
and effectiveness is a job placement consideration.
In addition to the direct services described above, the program offers
information in the form of presentations and workshops to the corporate sector and
community agencies. These workshops are designed to increase the sensitivity of the
participants regarding the needs of the deaf, hard of hearing and late deafened people.
They also address issues such as the integration of the deaf and/or hard of hearing people
into the workplace the application of ADA to prospective and current employees.
The Fair Lawn Deaf Program provides a valuable service to deaf/hard
of hearing people as well employees. Many individuals who are already in the job market
are able to have better work environments because of the interpreter/advocacy services
provided. The individuals who gain employment have the satisfaction of having been totally
involved in every phase of their job search, and the corporate world knows that expertise
and support provided by the Fair Lawn Deaf Program will help to ensure a successful job
placement.
The fair Lawn deaf Program has a long and proud history of providing
uncompromising service in assisting the Deaf, Hard of Hearing and Late Deafened Adults
determine their career goals, and then following a job search through to fruition.

What is Family Services of Westchester/Youth Forum?
Youth Forum is a peer support program for older adolescents and young adults ages 17-24
who have been involved in the mental health system including special education, juvenile
justice system, psychiatric hospitals, residential treatment services and substance abuse
services.
Youth Forum is a peer run program that provides support groups, recreation and
socialization. They also provide peer advocacy as well as education for
professionals and policy makers on what helps and what harms in the system.
The program has been successful for seven years in helping young adults succeed.
They participate in high risk local case conferencing networks in ten Westchester County
communities.

What is Family Voices?
Family Voices is a national grassroots network of volunteer parents speaking on behalf
of children with special health care needs. They provide free literature, state and
national newsletters, phone assistance (in Spanish as well), and they attend parent
meetings. In addition to individual assistance to families on health and related
education issues, Family Voices collaborates with other organizations advocating for
families, and working on policy change.
Family Voices was able to grow in New Jersey under a grant from the Robert Wood Johnson
Foundation which was matched by the Statewide Parent Advocacy Network and Special Child
Health Services of the Department of Health. They have done presentations on managed
care at state and national conferences and helped develop flyers for families on the
Medicaid Managed Care Alliance. Family Voices is involved with work incentives
through the SSI Alliance, acting as a liaison with Senator Toricelli on the Work
Incentives Improvement Act.

What is Fedcap Rehabilitation Services?
Fedcap's mission is to help people with disabilities and barriers to employment achieve
independence and full participation in the economic mainstream. Fedcap was founded in 1935
by individuals with severe disabilities at a time when people with disabilities were not
believed to be capable of employment or self-sufficiency. Fedcap was one of the first
vocational rehabilitation organizations in the country and helped to define the
profession.
Each year, over 3,000 New Yorkers trade dependency for economic self-sufficiency thanks
to Fedcap. Our training programs and support services give individuals the skills and
confidence they need to overcome barriers to employment. Over 90% of our trainees obtain
jobs at private companies or with Fedcap.
Ever day, Fedcap demonstrates that employing people with disabilities makes business
sense. With a workforce of 900 people at more than 40 locations, Fedcap is the largest
employer of people with disabilities in the region. Our workers deliver high quality
custodial, office, industrial and security services throughout the NYC area.

What is Forward Face?
Formed in 1978 and headquartered at the Institute of Reconstructive Plastic Surgery,
New York University Medical Center in New York City; Forward Face has evolved from a
self-help group to a charity for children with craniofacial conditions serving patients
being cared for at the IRPS as well as world-wide.
The primary goal of Forward Face is to help patients and their families find immediate
support and successfully cope with a craniofacial disorder. The extensive services Forward
Face provides include networking, educational materials, quarterly newsletters, and
financial aid. Trained volunteers provide hospital visits, home visits, telephone and
written communications to a full spectrum of patient population from newborns to
adults. Ongoing awareness programs and school presentations are integral elements in our
education campaign. Four annual meetings, two of which include presentations by medical
professionals and patients/families, give our members the opportunity to exchange ideas
and experiences, and to socialize. Inner Faces, a support group specifically for teenagers
and young adults, answers the needs of this important segment of our members. Featured
activities include communication workshops, theatre production, and social functions.
Future goals include acquiring a building which would offer rooms for lodging,
conference and meeting rooms, office, and library. Educational publications and videos are
continuously being expanded and updated.
Forward Face strongly advocates educating its members and the public in the quest for
understanding and acceptance. The pervasive effects and overwhelming demands a
craniofacial condition has on a person and their family's life requires comprehensive
support. Under the direction of The Board of Directors, Forward Face is totally committed
to providing the necessary resources to enable every person to attain the highest quality
of life.

What is Friends of the New Leaf, Inc.?
The Friends of the New Leaf, Inc. was established in order to enhance
mental health treatment of the serious and persistently mentally ill in
Columbia County through engaging clients in work activities. This stems from
the belief that work involvement builds self-confidence, self-image, and
provides an avenue for clients to transition from a status of societal
dependence toward a more independent level.
The vocational rehabilitation program runs an affirmative business (DBA), The
Office Deli OASIS. This business operates Monday through Friday and
delivers lunches to customers in the local Hudson Area.
Our program provides an environment where the consumers of services can
gain actual work experience. The expected outcome is for a consumer who has
had little or no employment experience to become exposed to the
responsibilities and rewards of being employed. Those who have had past
work experience can brush up on and learn new skills. For some this becomes
the stepping stone along the path to competitive employment.

What is Gateway Community Industries, Inc.?
Gateway Community Industries, Inc. a United Way agency, has been providing
rehabilitation services in Ulster and Dutchess Counties for Over 40 years. It is Gateway's
mission to assist people in acquiring, using and maintaining the skills and supports they
need to be successful and satisfied in their lives. Gateway accomplishes this mission by

Gateway's consumer population includes individuals with physical, psychiatric and

What is GMHC?
Gay Men's Health Crisis (GMHC) serves over 11,000 women, children and men infected and
affected with HIV. Its mission is to reduce the spread of HIV disease; to help people with
HIV/AIDS maintain and improve their health and independence; and to keep the prevention,
treatment and cure of HIV an urgent national and local priority. GMHC provides a
comprehensive continuum of services that include hot meals, legal representation,
nutritional counseling, client advocacy, substance use counseling, treatment education,
HIV prevention, HIV testing and public policy advocacy.
People with HIV/AIDS, family members, social workers, medical providers and friends can

What is Citiview Connections?
Citiview Connections is a clubhouse model program. Citiview Connections provides case
management, social recreational opportunities and vocational rehabilitation services to
adults with psychiatric disabilities (Axis 1 diagnosis).
As a clubhouse, clients are called members. Members participate in the routine
operation and maintenance of the program. This participation provides members with a voice
in the life of the clubhouse and equips them with integral skills for entry into the job
market. Another way members of our program are prepared for the job market is through the
Transitioned Employment (TE) Program. Members who participate in the TE gain the
experience of part-time employment in local companies.
Our program is open from 9:00 am until 6:00 pm on weekdays and 10:00am to 4:00 pm on
Saturdays. There is no charge to anyone who becomes a member of our program.

What are Goodwill Industries of Western New York, Inc.?
Goodwill Industries of Western New York, Inc. is one of 187 autonomous North American
Goodwills. Goodwill Industries of Western New York, Inc. has provided program services to
over 3000 individuals with barriers to employment since 1995. These services in
diagnostic evaluation and functional assessment, facility and community based employment
programs, employability skills training, workforce readiness training, adult basic
education, occupational training, and placement services leading to competitive
unsupported employment. An additional 2000 individuals have been provided social and
informational services during this same period of time, including linkages and referrals
to other community based service providers. Seventy-one percent of individuals
served in Goodwill's community employment programs are placed into unsubsidized employment
. Seventy percent of these placements are in full-time positions with a ninety day

What is Hawkins Center/ Disability and Work Project?
The Hawkins Center of Law and Services for People with Disabilities was founded as a
community-based non-profit organization in 1986. Our mission is to assist people
with disabilities in achieving financial, mental and physical health stability,
thereby allowing them to live independently, with integrity, and avoid the descent into
extreme poverty that can result from loss of health and employment. To achieve this
end, the Center provides an array of services to people with physical and /or mental
disabilities.
Our services focus on helping individuals obtain the basic financial and medical
support of Social Security's disability programs. They include individual legal
representation on SSI and SSDI claims, educational and training seminars, social support
services benefits and general legal advice, and policy advocacy at the local level.
We also provide comprehensive civil legal services to low income Contra Costa residents
living with HIV through the Legal Services Project.
Through our Disability & Work Project, we seek to encourage and assist individuals
who are interested in working to achieve their employment goals without unnecessarily

What is Heightened Independence and Progress (HIP):
Center for Independent Living?
HIP is a Center for Independent Living for people with all types of disabilities.
HIP is a CIL that offers a broad variety of services to people with disabilities to their
families and advocates and to all others interested in the progress and independence of
people with disabilities. HIP is governed by, and staffed with, people who
themselves have disabilities and who understand the needs of their peers. HIP is not
a residence; the "center" is a hub of activities that make "independent

What is the Helen Keller National Center?
The mission of the Helen Keller National Center (HKNC) is to facilitate a national,
coordinated effort to meet the social, rehabilitative and independent living needs of
America's deaf-blind population through demonstration of appropriate rehabilitative
training techniques. Our goal is to assist persons with deaf-blindness in becoming

What is the Helen Keller Services for The Blind?
Helen Keller Services for the Blind, Formerly the Industrial Home for the Blind, is one
of the oldest continuously operated non-profit vocational rehabilitation agency in the
United States. We help blind and visually impaired New Yorkers of all ages live as

What is Horizon Health Services: Vocational Program?
The Horizon Health Services: Vocational Program provides comprehensive vocational
abilities and interest testing, hands-on assessment, job preparation, job placement, and
post-entitlement continuing support services to Horizon clients and others referred from
other addictions and mental health providers. Clients are placed in meaningful and
permanent competitive employment settings paying livable wages. The Vocational
Program is a certified testing and training site for NYS Vocational and Educational

What is the Independent living Center of the Hudson Valley?
Our resources are available to people of all ages with all physical and mental
disabilities, their families, community businesses, organizations, and local government
agencies. Services are free of charge and there are no age limitations. The
Independent Living Center of the Hudson Valley (ILCHV) equips individuals with

What is the Institute for Community Living?
The mission of the Institute for Community Living, Inc. (ICL) is to assist people with
emotional and developmental disabilities who need opportunities to improve their quality
of life and to participate in community by providing the highest quality of services and
support.
ICL has developed an array of housing options with a current capacity to house up to
1400 adults with serious mental illness and 55 adults with mental
retardation/developmental disabilities. These housing options include short term
crisis residential services, transitional treatment oriented residences for adults with
co-occurring psychiatric and addictive disorders; large congregate residences; new
construction single room occupancy dwellings; and apartments, both in single and
scatter-site locations, for single adults as well as families.
Additionally, ICL operates a comprehensive system of community-based mental health
clinical treatment and rehabilitation options in Brooklyn that focus particularly on the
integration of medical and mental health care. This includes a diagnostic treatment
center and a specialized behavioral health home care program for people discharged from
inpatient units to their own homes.

Rehabilitation Services?
ICD is a private, not-for-profit comprehensive outpatient rehabilitation center
that provides job training and placement, medical rehabilitation, mental health and drug
and alcohol services, and learning disabilities services to persons with a broad range of
physical, emotional, cognitive, communicative, learning and substance abuse disabilities.
ICD Medical Rehabilitation Services provides treatment for physical and functional
problems of many kinds. Our staff of Board Certified physiatrists (physicians
specializing in rehabilitation), physical therapists and occupational therapists treat
persons experiencing temporary or permanent loss of function, mobility and/or pain due to
injury, accident, stroke, joint replacement surgery, head injury, surgery or
disease. ICD's 15,000 square foot gym offers computerized equipment, hydrotherapy
and private treatment areas. The Wheelchair Clinic focuses on the prescription and
fitting of mobility equipment and provides travel training. The Speech/Language
Department offers a wide range of remedial approaches to communication and speech

ICD is a private, not-for-profit comprehensive outpatient rehabilitation center

Jewish Vocational Service of MetroWest (JVS) is a non-profit, non-sectarian
organization serving the vocational and rehabilitation needs of the community since

What is the J F K Johnson Rehabilitation Institute?
The J F K Johnson Rehabilitation Institute provides rehabilitation services to all age
groups (infants to seniors). There is a pediatric rehabilitation program offering
diagnostic and therapeutic services, a pre-school program, and early education for
autistic children. We provide a full vocational rehabilitation program for
adolescents and adults which encompasses vocational evaluation, situational assessment,

What is The Korean American Association for Rehabilitation of the
Disabled?
The purpose for which the corporation is being formed is organize, operate, and
maintain a center for handicapped, with a primary focus on the needs of the
Korean-American community.
These members have previously been reluctant to seek outside assistance in matters
involving disabled family members, relatives and friends, due to largely cultural and
linguistic barriers.
However, such barriers can be remedied; whereby education, information and assistance
can be provided to Korean-American with disabilities in order to attainment fundamental
human needs and endeavors.
These include, but are not limited to employment, education, transportation, and
medical; as well as to assist Korean-American with disabilities in the attainment of a
sense of personal independence, optimal social integration; maintenance of funds of the
application of such income and the actualization of such purposes; for the well being of
the disabled Korean American population.
The above will also benefit society as a whole; and help this organization engage in
other lawful activities that are generally undertaken to further of these enumerated

What is The League for the Hard Of Hearing?
The League for the Hard of Hearing is the oldest non-profit, out-of-hospital
rehabilitation facility in the United States, serving over 22,000 adults, children, and
infants each year. Though its multi disciplinary approach, including medical services,
rehabilitation, education and counseling, the League improves the quality of life for
individuals who are hard of hearing and deaf. The career development department provides
educational/career counseling, pre-employment counseling, job development and placement,
post employment counseling, workplace awareness training, tutoring for vocational
enhancement, college preparatory readiness training and career mentoring. Services are
available Monday through Friday from 9am to 5pm, evenings and Saturdays, all by

What is Legal Services of New Jersey/SSI Project?
The SSI Project was initiated in cooperation with the New Jersey Division of Family
Development and Legal Service of New Jersey. Our mission is to assist people who
are potentially disabled through General Assistance or Temporary Assistance to needy
family recipients. The SSI Project of LSNJ represents claimants at the initial and
reconsideration levels of adjudication, as well as providing funds for consultative
medical examinations for those who have not access to treatment.

What is the Long Island Advocacy Center?
The Long Island Advocacy Center, Inc. is a private not for profit agency dedicated to
protecting the legal rights of students and the disabled.
The Long Island Advocacy Center offers....
- Students solutions to problems such as discipline, truancy, suspensions, residency
disputes and inappropriate placements.
- Individuals with Disabilities assistance in obtaining vocational rehabilitation
services, benefits and appropriate educational programs.
Through mediations, negotiations and representation, specially trained attorneys and
paraprofessionals ensure that individuals receive the maximum service to which they are
entitled under the law.
LIAC provides training sessions in relevant laws, regulations and advocacy skills to
family members, students, educators, probation officers and other professionals.
As a result of LIAC's intervention, individuals have achieved greater self-sufficiency.
Instances of juvenile delinquency and school drop-outs have been greatly reduced, saving
taxpayer dollars.

What is The Long Island Center For Independent Living, Inc.?
The Long Island Center For Independent Living, Inc. (LICIL) is a not-for-profit
organization whose primary mission is to promote self-sufficiency and motivation among
citizens of Nassau County who happen to have disabilities. Persons of all ages and across
all disability categories are encouraged to participate in its programs and services. As
part of a large network of Independent Living Centers (ILC's) operating throughout the
state and the nation, it is committed to the belief that people with disabilities have not
only the right, but the responsibility, to participate as fully as possible in each and
every realm of society, from political and social to educational and professional. Since
its inception in 1979, LICIL has held fast to the grass roots philosophy of the
Independent Living Movement which took hold in the 190's and 1970's as an expression of
the desire of persons with disabilities to be self-supporting and equal partners in the
American Dream.
To that end, LICIL works tirelessly to provide a host of services designed to foster
and enhance the capacity of individuals with disabilities to claim their rightful place in
a world where their voices have all too often gone unheard. Available services range from
peer counseling, benefits advisement, and advocacy to information and referral, equipment
loan and transportation, and public awareness/sensitivity training. In addition, LICIL
produces a quarterly newsletter entitled CENTERFOCUS, which highlights recent and upcoming
events of interest to consumers, as well as articles and information on trends in
assistive technology, benefits, and local politics.
