Listing of Civil Surgeons and Panel Physicians
The Division of Global Migration and Quarantine (DGMQ) is responsible for providing the Technical Instructions to civil surgeons and panel physicians. DGMQ does not maintain a listing of civil surgeons and/or panel physicians since civil surgeons are designated by District Directors of the U.S. Citizenship and Immigration Services (USCIS) and panel physicians are selected by the Department of State (DOS).
For Civil Surgeons
Information on civil surgeons in your area is available by contacting the USCIS at 1-800-375-5283 or by visiting the Civil Surgeon locater on the USCIS website.
Also, a current listing of civil surgeons for your area may be obtained by contacting your USCIS field office. Additional information concerning USCIS field offices and contact information for those offices is available at the USCIS website.
For Panel Physicians
A current listing of panel physicians for a specific U. S. Embassy or Consulate may be obtained by contacting the consular or visa section. Additional information concerning U. S. Embassies and Consulates, including contact information for these sections, is available at the DOS website.
New U.S. Department of State Forms for the Overseas Medical Examination of Immigrants and Refugees
The obsolete Optional Form-157 (OF-157) has been replaced with the new DS-2053 and associated worksheets (DS-3024, DS-3025, and DS-3026). These comply with the changes in immigration law and the current Technical Instructions for Medical Examination of Aliens (June 1991, Changes July 1992, Addendum April 1997). They provide information for the resettling health departments and the Centers for Disease Control and Prevention about the immigrant or refugee.
