Non-personal Information We Record

On one of our Web pages, Get Involved Today!, we offer an interactive form that lets you voluntarily submit personal information (such as your e-mail address, name, mailing address, or organization). This occurs when you are requesting a publication, educational updates, or you wish to be notified of pertinent educational activities or events taking place in your area. In this case, all submitted information is used only for the expressed purposes for which it is intended and is not made available to any third party. However, when you submit a comment to the White House Initiative through its Web site, if you supply your name, e-mail address, or other personal information, that information will be received along with your comment.

White House Initiative on Educational Excellence for Hispanic Americans Get Involved Today! Form

The Privacy Act of 1974 requires that each federal agency that asks for information must tell you the following:

  2. How it will be used.
  3. What could happen if you do not give it.
  4. Its legal right to ask for the information and whether the law says you must give it.
  5. How to contact us.

We Collect this Information for the Following Purposes

The purpose of the information collected by the Get Involved Today! form is to ensure you receive the educational information, notices, newsletter, or publication you requested by mail or e-mail in a timely manner. Another purpose of the information collected by the Get Involved Today! form is to help the White House Initiative target its mailings to groups with specific educational interests, such as parents, students, educators, and community representatives. The Get Involved Today! form collects no privacy information.

It should take you approximately two to three minutes to complete the form. Completion of the form is entirely voluntary. If you don't complete it, you won't be denied access to any information available on the web site.

How We Will Use the Information We Collect

We will use the information collected when you complete the Get Involved Today! form or submit an e-mail request to send you educational information, updates, and notices. We will use the group or state designation to get a general idea of the affiliation or geographic distribution of our users. In addition, we will use the information collected when you complete the Get Involved Today! form to conduct individual, group, and mass mailings of useful educational information.

If You Do Not Provide All or Part of the Requested Information

If you do not provide information for the required fields on the Get Involved Today! form, the system will not record your mailing information and you will not be able to receive timely educational information, updates, or notices. Note that it is not necessary to complete the Get Involved Today! form to access the White House Initiative's Web content and services.

You are not required to provide your real name.

If you elect to receive e-mail updates, you must provide a valid e-mail address or the e-mail updates cannot be delivered to you.

Our Authority to Collect the Information

The White House Initiative obtained clearance from OMB for the collection of information through its White House Initiative on Educational Excellence for Hispanic Americans web page. This approval was sought under an existing generic OMB control number granted previously to the Department of Education. OMB approved the collection under the Customer Satisfaction Survey process.

Systems of Records Notice

The general authority for the system of records used to store information you elect to submit is Executive Order 13230, 66 FR 52841. When you submit information on the Get Involved Today! form, we are authorized to maintain a record of your personal information (such as your e-mail address, name, mailing address, or organization. Detailed information is available in the White House Initiative Partners in Education (System Number 18-06-05).

The Paperwork Reduction Act of 1995

Under the Paperwork Reduction Act of 1995, you are not required to complete a government form unless it displays a valid OMB control number. The OMB control number for the White House Initiative on Hispanic Americans Form is 1800-0011.
