Investigator Registration Packet

All data fields on each of the following forms must be completed, signed and submitted simultaneously to finalize your investigator registration. In addition to these forms, a current CV must be included in your submission. Each of the following three forms must be completed and submitted with a copy of your CV to the Pharmaceutical Management Branch (PMB)

• Investigator Registration Form (FDA 1572) (PDF)
• Supplemental Form (PDF)
  Additional information that will be used for CTSU and Group membership
• Financial Disclosure Form (PDF)
  Disclosure Form to report any involvement in agent marketing
