CRIS Accounts

How do I get a CRIS account?

  1. All staff (except Physicians and other Prescribers) must submit a CRIS Access Request FormPDF Logo.
  2. All staff must schedule and complete CRIS training. See Class Information on available courses and how to register.
  3. All staff must personally deliver completed documents to the DCRI Office, Building 10, Room 1C290.  Please bring your NIH ID badge.
  4. CRIS codes will be distributed at the end of training provided all appropriate forms are submitted and approved in advance.  If not, the staff member will be responsible for picking up their CRIS code at a later date.  Codes will not be distributed without personal identification so bring either your NIH ID badge, other U.S. Government ID or Drivers License.

Note: Physician/Prescriber codes are issued by the Office of Credentialing Services (301-496-5937). Completion of CRIS training is required prior to obtaining a CRIS code.
