Administrative Staff (Non-HR)
Complete the following forms and send them to HR Systems Support, EPS/100:
1. Security Authorization Form (.pdf)
2. Access Disclosure Form (.doc) (Only needed if you do not have one on file for other systems access)
3. In addition to the forms above contract staff must complete the Contractor Access Agreement Form (.pdf)
