::. Import Permits and Export Declarations

Note:

Each time biological, diagnostic or infectious materials are imported or transferred, an Application for Permit to Import or Transport Agents or Vectors of Human Disease (CDC Form 75-3) permit must be submitted and approved by the NIH Quarantine Permit Service Office (QPSO). When shipping biological, diagnostic or infectious material outside the NIH facilities, a Declaration for Exportation of Biological Materials (NIH form 2388) must be signed by the QPSO. All packaging must comply with the Department of Transportation (DOT), the Federal Aviation Authority (FAA) and the International Air Transport Association (IATA) dangerous goods regulations. Federal and international regulations require that the packager successfully complete job-specific training and be a certified shipper before shipping biological, diagnostic or infectious materials for transport.
