Community Health Promotion Handbook: Learn more about this handbook and download a copy This publication was developed through a collaborative effort between CDC's Steps Program and Partnership for Prevention to provide implementation guidance ("Action Guides") on the following five effective community-level health promotion strategies: 1) Establishing a diabetes self-management education program, 2) Developing a community trail, 3) Increasing physical activity in schools, 4) Developing a walking group program, and 5) Improving tobacco-use treatment in healthcare delivery systems.

MMWR Surveillance Summary: HTML version | PDF version (1.87MB) This MMWR report summarizes the results of surveys conducted in 2007 of high school students in 26 communities participating in CDC's Steps Program. As a component of the Youth Risk Behavior Surveillance System (YRBSS), communities also conducted school-based surveys of students in grades 9–12 in their program intervention areas. The communities used a modified core questionnaire that measured dietary behaviors, physical activity, tobacco use, and the prevalence of overweight, obesity, and asthma.
