About the Bemidji Area
The Bemidji Area Indian Health Service is an agency within the Department of Health and Human Services whose mission is to raise the physical, mental, social and spiritual health of American Indians and Alaskan Natives (AI/AN).

The Bemidji Area Indian Health Service administrative office is located in Bemidji, Minnesota. The service delivery area includes AI/AN in five states, Michigan, Minnesota, Wisconsin, Illinois and Indiana along with five urban programs. Within the region, there are currently 35-plus federally recognized tribes. The total population served by the Bemidji Area Indian Health Service exceeds 110,000 individual patients.


Minnesota

Cass Lake PHS/IHS Hosptial (Federal)
The 13-bed Cass Lake PHS Hospital, with a staff of +130 staff, provides inpatient and outpatient services including family medicine, internal medicine, nursing staff, pediatrics, sing bed, emergency room, urgent care, pharmacy, dentistry, optometry, physical therapy, radiology, laboratory, audiology and diabetes management and education.

Red Lake PHS/IHS Hospital (Federal)
The Red Lake staff consists of about +145 full time staff and about 100 supplemental health care employees. Service includes a 23-bed hospital, family and internal medicine, emergency room, nursing staff. Specialty clinics (diabetes, hypertension and rheumatology), radiology, lab, dentistry, pharmacy, health education, physical and occupational therapy, audiology, mental health care and dietary planning & education.

White Earth PHS/IHS Clinic (Federal)
White Earth Health Center includes family and internal medicine, podiatry, dentistry, optometry, mental health care, physical therapy, lab, physical therapy, health education, nutrition/dietetics and pharmacy. White Earth is staffed by +100 full time employees. Inpatient services are referred to local private hospitals.

Bois Forte Clinic (Tribal)
The staff of 40 includes dentistry, podiatry, pharmacy, women's health specialist, audiology, community health representatives and community health nursing.

Fond du Lac Human Services (Tribal)
The health staff included family practice physicians, nursing, 'mid level' staff, dentistry, pharmacy, public health nursing, social work and mental health. Inpatient care is contracted with local private hospitals in both Cloquet and Duluth.

Grand Portage Clinic (Tribal)
The staff at the Grand Portage clinic includes nursing and community health representatives. Services include general nursing services and immunizations. Inpatient care is referred to neighboring hospitals.

Lower Sioux Clinic (Tribal)
Through contracts with medical and dental providers in the surrounding area, the Lower Sioux Indian Community Center offers dentistry, optometry, pharmacy, diabetes care and community health nursing. Patients who need inpatient care are referred to local community hospitals.

Mille Lacs Health Center (Tribal)
The combined efforts of three clinics provide family medicine, dentistry, podiatry, pharmacy, optometry, audiology, public and mental health and family services. Inpatient services are referred to specialists throughout the area.

Prairie Island Community Clinic (Tribal)
The Prairie Island Community Clinic offers family medicine, diabetes care, podiatry, nutrition and fitness education and training. Patients who need inpatient care are referred to local community hospitals.

Shakopee Dakota Clinic (Tribal)
The Shakopee Dakota Clinic is staffed by nurse practitioners, RNs, dental care, mental health (social work, psychologist, chemical dependency and family advocates). Services mainly include family medicine. Inpatient services are contracted through area facilities.

Upper Sioux Community Health Services Clinic (Tribal)
The Tribal health clinic offers dentistry, optometry, pharmacy, mental health and diabetes care through local contracts with local providers. Health education, immunization clinics and fitness training are available at the tribal facility.

Wisconsin

Bad River Health Services (Tribal)
The Bad River Clinic is staffed by two physicians, two nurse practitioners, a community health nurse, two podiatrist, a pharmacists, three mental health counselors, two alcohol and other drug abuse counselors, a dietician and two community health representatives. Inpatient services are provided by area medical centers.

Forest County Potawatomi Health and Wellness Clinic (Tribal)
The center is a modern clinic with over 80 full time employees. Some but not all of the services offered are family medicine, internal medicine, pediatrics, obstetrics/gynecology, psychiatry, rehabilitation services, dentistry, orthodontics, podiatry, lab, radiology, optometry, chiropractic, pharmacy and mental health. Inpatient care is provided at area hospitals.

Ho-Chunk Health Care Clinic and House of Wellness Clinic (Tribal)
The Ho-Chunk Health Care center is staffed by two physicians, two nurse practitioners, three nurses a nurse educator, two medical technologists, an x-ray technologist, a dentist, dental hygienist, pharmacist, optician, mental health counselor and chemical dependency counselor.
The House of Wellness Clinic is staffed by three physicians, a podiatrist, family nurse practitioner, two clinical nurses, a nurse educator, pharmacist, dentist, optician, mental health counselor and a chemical dependency counselor. Inpatient services for both clinics are referred to local hospitals.

Lac Courte Oreilles Clinic (Tribal)
The health and urgent care is a full-service ambulatory center. Over 40 individuals are employed at the center and services include family medicine, dentistry, optometry, radiology, laboratory, pharmacy, alcohol and other drug abuse clinic, mental health care, community health and urgent care. Inpatient care is provided at local hospitals.

Lac du Flambeau/Peter Christensen Health Center (Tribal)
The staff at the Peter Christensen Health Center includes two dentists, a dental hygienist, three dental assistants, a contracted podiatrist, a pharmacist, two pharmacy technicians, women's health specialist, a contracted audiologist and three community health representatives, and a community health nurse. The clinic refers patients to area hospitals for inpatient care.

Menominee Clinic (Tribal)
The Menominee Tribal Clinic's services include family medicine, dentistry, optometry, pharmacy, community health and 24/7 medical emergency care. A staff of six family physicians and two nurse practitioners services over 4,0000 Menominee people. Inpatient services are provided by local hospitals.

Oneida Community Health Center (Tribal)
The Oneida Community Health Center serves over 22,000 Native Americans each year. The 65,405 square foot facility has 36 exam rooms with the ability to expand to over 50. The 175 full and part time employees offer an impressive list of medical, dental, technical and mental health services. Inpatient services are provided at four local hospitals in the Green Bay area.

Red Cliff Community Health Center (Tribal)
The health center is staffed by a physician, nurse practitioner, pediatrician, dentist, two pharmacists, three nurses and a clinical social worker. Services offered include family medicine, dentistry, radiology, laboratory, pharmacy, mental health clinic, community health and an alcohol and other drug abuse clinic. Inpatient is provided by area hospitals.

Sokaogon Chippewa Health Clinic
The clinic offers family and internal medicine, podiatry, mental health care, pharmacy and an alcohol and other drug abuse services. The clinic is staffed by two physicians, a physician assistant, a nurse practitioner, nursing and podiatry. Inpatient care is provided by area hospitals in the surrounding area.

Stockbridge-Munsee Health and Wellness Center (Tribal)
The staff at the center includes three physicians, a nurse practitioner, two dentists, a dental hygienist, two pharmacists and assistants, a chiropractor, clinical psychologist, social workers, counselors and various contract professionals. A plethora of services are offered. Inpatient serves are admitted to local hospitals.

St. Croix Health Clinic (Tribal)
The St. Croix Tribal health Department provides family medicine, dentistry, optometry, pharmacy, mental health care, an alcohol and other drug abuse clinic, nutritional services, community outreach and diabetic education to name a few. The health care staff includes a physician, dentist, optometrist, a pharmacist, registered dieticians, counselors and several nurses.

Michigan

Bay Mills Health Center/Ellen Marshall Memorial Center (Tribal)
The center is staffed by a physician, nurse practitioner, three nurses, two dentists three dental assistants, dental lab tech, dental hygienist, psychologist, substance abuse counselors and three community health registered nurses. Inpatient services are contracted through nearby towns.

Grand Traverse Band Family Health Clinic (Tribal)
There are 16 employees won staff, six of which are outreach staff working outside the clinic. One family practitioner is employed at the clinic as well as three nurses, two community health nurses and four community health representatives. Inpatient care is referred to local communities.

Hannahville Health Center (Tribal)
The center is staffed by a contract physician, three registered nurses, a dentist and a pharmacist. Services includes family medicine, dentistry, pharmacy and mental health care. Inpatient services are contracted through area facilities.

Keweenaw Bay Indian Community Clinic (Tribal)
The service's clinic is staffed by two physician and a pediatrician. The dental clinic is staffed by a dentist, two dental assistants and a full time dental hygienist. Other services employ a clinical psychologist, two social workers, a certified diabetes educator and a nurse. Inpatient and ER services are provided at the local hospital.

Lac Vieux Desert Health Center (Tribal)
The Lac Vieux Health Centers' services include family medicine, dentistry, optometry, pharmacy and mental health care. On staff at the clinic are a physician, family nurse practitioner, two registered nurses.

Little River Band Clinic (Tribal)
There are 20 employees on staff at the Little River Band of Tribal health Services. Services are administered by the tribe and include family medicine, community health resources, community health nursing, diabetes education, behavioral health care, substance abuse counseling and contract health services. Patients needing inpatient care are referred to area hospitals.

Little Traverse Bay Band Clinic (Tribal)
The health center's 22 employees include two physicians, two physician assistants, five registered nurses, a medical assistant, three community outreach representatives and a transporter. They provide family medicine, women's medicine, pediatrics and community health care. Inpatient services are provided through northern Michigan hospitals.

Match-e-be-nash-she-wish Potawatomi Clinic (Tribal)
The Health and Human Services currently employs eight staff within the following departments: community health nursing, diabetes program, community health representative program, contract health services, behavioral health services and human services coordination. Patients needing inpatient care area referred to hospitals in surrounding areas.

Nimkee Memorial Wellness Center (Tribal)
The Nimkee Memorial Wellness Center is staffed by five medical providers, two dentists and several employees in the public health department. Services include family health, dentistry, men's and women's wellness clinics, diabetes program, maternal/child health care and women, Infants and Children Program. Inpatient services are contracted with are providers.

Nottawaseppi Huron Band Clinic (Tribal)
The Huron Potawatomi Health Department operates to facilities. The collective energies of these two ceners provide family medicine, diabetes care, nutrition services, community health caer, behavioral health care and nutirion services.

Pokagon Potawatomi Health Clinic (Tribal)
Though located in Michigan, this facility serves American Indians located in Indiana as well. There are 20 individuals on staff at the Pokagon Potawatomi Health Center. They offer family medicine, pharmacy, mental health care, substance abuse counseling and nutrition services. Outpatient services are contracted through the local hospital and other facilities.

Sault Ste Marie Clinic (Tribal)
There are over 125 staff members employed at the main center and an additional 100 staf members serve the area through satellite clinics. Staffing consists of about 90 health care professionals including six physicians, two physician assistants and four nurse practitioners. Services include family medicine, dentistry, pharmacy just to name a few. Inpatient care is provided/contracted through local and area hospitals.

Urban Programs

Chicago- American Indian Health Services
The staff of 12 include two full time administrators, a full time diabetes nurse and two full time community health workers/administrative assistants. Part time staff include a licensed counselor, psychologist, a physician and family nurse practitioner. The outpatient facility provides diabetes care and also behavioral health care.

Detroit - American Indian Health and Family Services of SE Michigan
With a staff of 25 and fice contract staff employees, the American Indian Health and family Service of SE Michigan offers family medicine, dentistry, behavioral health care, substance abuse care, youth preventative tobacco and alcohol awareness. Native American tradition medicine and teachings.

Milwaukee - Gerald Ignace Health Center
This center is located in Milwaukee and has a staff of 25. Services at the Gerlad L. Ignace health Center include family medicine, pediatrics, internal medicine, obstetrics/gynecology, dental care, lab, pharmacy, comprehensive diabetes care, mental health and chemical dependency programs

Minneapolis Indian Health Board
There are roughly 65 employees on staff at the center. Services include family medicine, dentistry, mental health care, outreach program transportation program 340B Drug Discount Program and Women, Infant & Children Programs. Patients needing inpatient care are referred to nearby hospitals.

Green Bay United Amerindian Center, Inc.
The center has 12 employees on staff including nursing, various counselors and administration. Services include mental health care, chemical dependency program and transportation to health care facilities.
