Grant and Cooperative Agreement (Financial Assistance) Forms and Regulations

Forms

Entities applying for financial assistance (awarded as a grant or cooperative agreement) need to submit application forms to NRCS. The Standard Form (SF) 424, Application for Federal Assistance, is required from every applicant. Two additional forms, determined on the basis of whether the project is for construction or non-construction, must also be submitted:

SF 424A Budget Information -- Non-Construction Programs
SF 424B Assurances -- Non-Construction Programs

Required for a construction project:

SF 424 Application for Federal Assistance
SF 424C Budget Information -- Construction Programs
SF 424D Assurances -- Construction Programs

After award of a grant or cooperative agreement, financial assistance recipients will use the following forms, as explained in the grant or cooperative agreement, to request reimbursement and report the financial status of a project:

SF 270 Request for Advance or Reimbursement - used to request payment for work completed under a non-construction  program.
SF 271 Outlay Report and Request for a Reimbursement for Construction Programs -- used to request payment for work completed under a construction programs.
SF 272 Federal Cash Transactions Report -- used to report advances.
SF 269A Financial Status Report (Short Form) -- this form is submitted to NRCS the project is completed.  The form summarizes financial activity during performance of the project.

Regulations

The following regulations and policies must be followed by recipients accepting financial assistance from NRCS. These regulations are published in Titles 2 and 7 of the Code of Federal Regulations (CFR) and in Office of Management and Budget Circulars. Several of the regulations apply uniformly to all recipients; others apply based on recipient category as follows:
