Your Health Career Starts Here - IHS Scholarship Program

Your completed application, including all required documentation, must be submitted by the following deadlines:

Continuation Students can now apply online! Application and documentation to the IHS Division of Grants Management Office, postmarked no later than February 28, 2009.

New Applicants - Application and documentation to your Area Scholarship Coordinator's office, postmarked no later than March 28, 2009.

Instructions for Application

There are two ways for you to begin the application process. The preferred application is the online application. You may also consult your Area Scholarship Coordinator or complete the Contact Us form to request that a printed Scholarship Application Handbook be mailed to you.

The online process consists of 12 steps that take the place of the following four hardcopy forms from the Application Handbook:

1. Application Form (IHS-856, also referred to as the bubble sheet)

2. Delinquent Federal Debt Form (IHS-856-6) - which outlines your financial obligation and information. (This form is a little different from the rest of the online application. In addition to answering the online questions, you will also need to provide a signed paper version of this form. You can find the PDF file under the Forms and Handbooks section of this site.)

3. Reasons for Requesting Scholarship Form (IHS-856-5, also referred to as the narrative)

4. Faculty/Employer Evaluation Form (IHS-856-4)
