This section includes sample tools and resources
that are available for groups’ use while conducting
the NPHPSP assessment. Many of these resources are
available for download and customization.

Ground
Rules - an example of ground rules
that have been used in association with the
retreat / small group approach.
Voting Cards - cards that can
be printed to develop “voting cards” for the
assessment process. Each response option can
be printed on different colored paper or cardboard
strips.
Response Options Poster - a graphic
that can be printed onto a large piece of paper
to produce a poster displaying the response
options.
Evaluation Forms

Data and Accessing Reports - detailed
instructions on submitting data and accessing
reports.
Scoring methodology – for those that
are interested, these documents provide an overview
of how data is analyzed and scored once it is
entered into the system. Three different documents
are available: a
complete scoring methodology which describes
the full process and exceptions; an
abbreviated
and lay-friendly one-page description; and
an even shorter
paragraph description.
Sample Reports – provides samples
of the reports that users receive after entering
data. All users can download their results and
are able to view four files: a narrative; summary
data, charts and graphs; and scored and raw
data in Excel and comma-separated value (CSV)
format. The following samples are available
