Joslin's Web-based Diabetes Registry and Risk Stratification System

This system is based on evidence-based clinical guidelines and helps providers identify and intervene with patients who are most likely to develop costly and debilitating diabetes-related complications. The system stratifies patients by risk and recommends care management and education interventions at point-of-care. Reports provide data for benchmarking and making regional comparisons for all of the key diabetes-related performance measures commonly recognized by Health Plan Employer Data and Information Set (HEDIS) and the American Medical Association (AMA). The program reports improvement in a variety of process and outcome measures.11

The Community Chronic Care Network is funded by the Agency for Healthcare Research and Quality from October 2004 to September 2007 to implement an electronic registry tool to ensure that all people with diabetes care in Santa Cruz County, Calif. receive consistent and continually improving medical support.
