artwork, even storage space
- Examples of Successful Partnerships
The Task Force on Community Preventive Services is a 15 member
non-Federal task force supported by the Centers for Disease Control
and Prevention (CDC). To improve the health of people with diabetes,
the Task Force reviewed the evidence of the effectiveness of diabetes
disease and case management and self-management education. The
Guide to Community Preventive Services reviews interventions that
occur outside traditional clinical settings, such as the home
and faith institutions. Details of this review and the final recommendations
