Planned care visit video clip

Improving Chronic Illness Care has developed a videotape of a nurse practitioner demonstrating patient-centered self-management education. The time it takes the nurse practitioner to do the illustrated tasks (initial patient conversation, telephone follow-up, etc) demonstrates that it does not take much time for health care providers to promote self-management effectively. The planned care visit video contains three segments.  

1) The Patient Experience, time 16:04: walks viewers through a planned care visit from the initial outreach call (based on identifying the patient from a registry), through the physician visit and follow-up with the team nurse.
Audience: Clinicians and administrators

2) The Provider Point of View, time 5:00: shows the activities of the care team emphasizing the preparation, teamwork and communication to provide guideline-driven care and to support the patient in managing her condition on a daily basis.
Audience: Clinicians and administrators

3) The Self-Management Interview, time 12:07: demonstrates the use of collaborative goal setting, creating an action plan, and problem solving. Subtitles are used to highlight useful techniques for engaging and supporting patients and teaching self-management skills, including motivational interviewing.
Audience: Clinicians who help patients do in-depth self-management planning.
