The Institute of Medicine provides an excellent guide, Improving Health in the Community* that describes the use of community indicators and performance monitoring to improve community health. The guide also provides tools to help communities develop their own performance indicators. CDC's Healthy Days Measures are included among the IOM's suggested community performance indicators.

The State of Aging and Health in America (2004) (PDF–1.2Mb)
A National and State-by-State Report Card on Healthy Aging with 15 indicators.

United Way State of Caring Index (2004)*
A quality of life barometer that incorporates 36 indicators covering 1992-2002, including financial security, health, education, safety, charitable giving, volunteerism, civic engagement and the natural environment. It was designed to measure social, economic, and environmental health at the national and state levels.
