Required Identity Documentation for Driver License, Permit and ID Card Transactions
Every time you visit a DMV office to conduct business regarding a driver license, driver permit or identification card, you are required to bring proof of your full legal name, your legal presence in the U.S., your identity, your date of birth, and your Social Security Number (SSN). Also, if your address has changed since you were last issued a card, you will also need to bring proof of your Oregon residence address.

Proof of Your Full Legal Name
Your full legal name is your first name, middle name(s), and last or surname, without use of initials or nicknames.
 
You must provide additional documentation proving your current full legal name if it is different than the name shown on your proof of legal presence (See the Proof of Your Legal Presence, Identity and Date of Birth section below for more information).
 
Acceptable documents that can be presented to prove your current full legal name include:
  • An official government issued marriage certificate/license (signed by a government official and including a filed date, stamp, seal or other notation showing that the document has been filed with a government agency);
  • A certificate of Registered Domestic Partnership issued by a city, county or state Vital Statistics agency;
  • A U.S. city, county or state court-issued divorce decree, judgment of dissolution of marriage, annulment of marriage decree, judgment of dissolution of domestic partnership, or annulment of domestic partnership;
  • A government-issued death certificate of spouse, that includes a connection to your current full legal name (signed by a government official and including a stamp to show that the document has been filed);
  • A U.S. city, county or state court-issued adoption decree;
  • A U.S. city, county or state court-issued legal name change decree; 
  • A U.S. city, county or state court-issued custody decree or guardianship decree;
  • Oregon driver license, instruction permit or identification card;
  • Canadian government issued birth certificate;
  • Military ID card, Common Access card and Uniform Services ID & Privilege card (including all branches of military personnel and dependents, not including Merchant Marines);
  • Other U.S. state, U.S. territory, District of Columbia, Canadian or U.S. Department of State driver license, instruction permit or identification card;
  • Oregon Concealed Weapon permit/Concealed Handgun license;
  • Tribal identification card issued by a federally recognized tribe located in Oregon or with an Oregon affiliation, approved by DMV;
  • Oregon County Community Corrections, U.S. Pretrial, U.S. District Court Probation Office, or Oregon Youth Authority proof of identification letter, approved by DMV.
All documents presented must be original or certified copies from the issuing agency to be accepted by DMV.  DMV has the discretion to reject or require additional evidence to verify your current full legal name.
 
If you are a citizen of, a permanent resident of or legally present in the U.S. and do not have the required documents needed to prove your current full legal name you may be eligible for an Oregon temporary driver license or identification card.

Proof of Your Legal Presence, Identity and Date of Birth
You are required to provide proof of your legal presence in the U.S., your identity and your date of birth. 
 
Legal presence means that a person is a U.S. citizen, permanent legal resident, or otherwise legally present in the U.S.
 
One of the following documents is required when applying for an original, renewal or replacement driver license, driver permit or identification card:
  • Birth document issued by a city, county or state in the U.S., District of Columbia, Census Bureau or a US Territory (American Samoa, Puerto Rico, Guam, U.S. Virgin Islands, Northern Mariana Islands and Canal Zone):
    • The document cannot be laminated.
    • DMV will not accept birth documents issued by a non-government agency, such as a hospital issued birth certificate, hospital issued birth card, hospital issued birth registration or a baptismal certificate.
  • A U.S. Consular Report of Birth Abroad (FS-240).
  • A U.S. Government issued Certification of Birth (DS-1350 or FS-545).
  • A Request for Verification of Birth (DD372) that must include date/signature of recruiting officer, and signature, date and official seal or stamp of the issuing Vital record agency.
  • Report of Child Born Abroad of American Parent(s).
  • A valid U.S. Passport, Passport Card or Emergency Passport expired no more than 5 years.
  • A valid Federated States of Micronesia (FSM) or Republic of the Marshall Islands (RMI) or Republic of Palau Passport expired no more than 5 years, must include Arrival/Departure Record (I-94).
  • A tribal ID card issued by one of the following:
    •  Confederated Tribe of Warm Springs Reservation of Oregon;
    •  Confederated Tribes of Islets Indians;
    • Confederated Tribes of Umatilla Indian Reservation;
    • Burns Paiute Reservation;
    • Cow Creek Band of Umpqua Indians;
    • Official Tribal Identification of the Klamath Tribes; or
    • Confederated Tribes of Grande Ronde Oregon.
  • A valid foreign passport not expired and includes one of the following:
    • Arrival/Departure Record (I-94, CBP I-94A); or
    • A page within the passport stamped “Processed for I-551”; or
    • Notice of Action (I-797A), (NOTE: The I-797 is the receipt received showing that you have applied for a change or extension of your legal presence. DMV will only accept the I-797A approving the change requested as proof of your legal presence.); or 
    • I-94 or CBP I-94A stamped “Refugee” or “Asylee”; or 
    • I-94, CBP I-94 or CBP I-94A stamped “Paroled Until” or “Parolee”.
  • I-94 with attached photo stamped “Upon Endorsement serves as Temporary I-551 evidencing permanent residency for one year.
  • One of the following U.S. Department of Homeland Security documents, not expired:
    • Temporary Resident Identification Card (I-688);
    • Employment Authorization Card (I-688A, I-688B, and I-766);
    • Certificate of Citizenship;
    • Resident Alien Card or Permanent Resident Card;
    • Certificate of Naturalization;
    • Permit to Re-enter (I-327);
    • Refugee Travel Document (I-571);
    • Identification Card of Resident Citizens in the United States (I-179); or
    • U.S. Citizen Identification Card (I-197). 
All documents submitted must be original or certified copies issued by a federal, state or local government agency of the United States, with the exception of a foreign passport being presented with an acceptable immigration document, and must be acceptable to DMV.  DMV has the discretion to reject or to require additional evidence to verify your legal presence in the U.S., your identity and date of birth.
 
Your full legal name is the name shown on your legal presence/identity document. If your current name is different than the name shown on your legal presence/identity document, you will be required to provide additional documents proving your current legal name (see the Proof of Your Full Legal Name section above for more information).  
 
If your date of birth is incorrect on your legal presence/identity document you will be required to have the document corrected before it will be accepted as proof of your legal presence, full legal name, identity and/or date of birth.

Proof of Your Social Security Number
You are required to provide proof of your Social Security Number (SSN) or proof of your ineligibility for one. DMV verifies numbers with the Social Security Administration (SSA). If your SSN fails the verification process, you will have to resolve the issue with the Social Security Administration (SSA) before DMV will be able to issue, renew or replace your card or permit.
 
Acceptable proof of a Social Security Number includes:
  • A Social Security Card or other SSA documentation, which may be laminated (metal souvenir cards and photocopies are not acceptable);
  • A copy of a state or federal tax document;
  • An employment record containing the SSN;
  • A military document containing the SSN;
  • A document from the Social Security Administration validating the SSN, such as a benefits letter;
  • An out-of-state driver license, permit or ID card containing the SSN;
  • Tribal ID Card containing the SSN;
  • Medical Benefits Card containing the SSN; or 
  • Any acceptable document submitted as proof of legal presence/identity or residence address containing the SSN.
Acceptable proof of ineligibility for a Social Security Number include:
  • A document issued by the Social Security Administration;
  • A document issued by a Federal agency or Federal court which demonstrates the customer is ineligible for an SSN;
  • An I-94, CBP I-94, CBP I-94A without the stamp noting “Employment Authorized”; or
  • IRS Individual Tax Identification Number (ITIN).
Note: You may qualify for a temporary driving privilege or identification card if you are working with other agencies to obtain the required documents.
 
Why is DMV collecting SSNs?
The Federal Welfare Reform Act includes the requirement that each state collect the Social Security Numbers of all drivers to assist in the enforcement of child support laws.
 
Anyone who has been issued an SSN by the Social Security Administration will be required to provide it to DMV (OAR 735-062-0005). DMV will not issue, renew, or replace a driver permit, driver license or identification card unless you provide proof of your SSN. Customers who are not eligible for an SSN will be required to provide proof that they are not eligible. Customers who refuse to provide proof of, disclose their SSN, have never been issued an SSN, or refuse to provide proof that they are not eligible for an SSN will not be issued an Oregon driving privilege or identification card.
 
DMV will verify numbers with the Social Security Administration (SSA). If the SSN does not match the SSA record, the customer will be required to resolve the issue prior to DMV issuing, renewing or replacing a driver license, driver permit or identification card.
 
Federal regulations require that applicants for a commercial driver license (CDL) or CDL instruction permit have a verifiable SSN, no exceptions.
 
What if I don't have a SSN?
If you have never been issued a number by the Social Security Administration (SSA) and are eligible for an SSN, you will be required to obtain a SSN from the SSA before DMV will issue an Oregon driving privilege or identification card.
 
If you are not eligible for a SSN, you must provide proof that you are ineligible and sign a statement of non-eligibility certifying that you are in fact ineligible.
 
Who will see my SSN?
DMV takes the responsibility to safeguard personal information very seriously. DMV employees receive training on how to safeguard records and are required to follow state and federal privacy laws. Your SSN will not be displayed on your driver license, driver permit or identification card, or released on DMV records. Access to your SSN is allowed only to qualified government agencies.
 

Proof of Your Oregon Residence Address
You must provide proof of your Oregon residence address if:
  • This is your first time applying for an Oregon driving privilege or identification card; or
  • Your address has changed since the last time you renewed or replaced your current driver license, driver permit or identification card.
You must provide at least one document from the Acceptable proof of residence documents listed below.  Documents must:
  • Be originals or NCR (carbonless) copies where applicable;
  • Contain both your name and address (not acceptable if addressed "in care of," "parent of" or "for";
  • Be in ink, typed or computer/printer generated; and
  • Contain no apparent alterations, such as erasures, different/varying handwriting, information crossed out, overstrikes, correction liquid or tape.
Acceptable proof of residence documents include:
  • An official government issued marriage certificate/license including city, county or state (signed by a government official and including a filed date, stamp, seal or other notation showing that the document has been filed with a government agency) containing the current residence address;
  • Mortgage documents;
  • Oregon DMV address sticker with residence address only, the sticker will not be accepted if it contains a mailing address.
  • A statement from any person residing at the same residence address as the applicant. The person making the statement must accompany the applicant and present at least one acceptable document proving proof of current residence address;
  • A utility statement of account, hook up or work order dated within 60 days of the application;
  • Payment booklet;
  • Oregon vehicle title or registration documents containing only residence address
    • If the document contains a mailing address the document will not be accepted;
    • If using a vehicle registration/title you must be the primary owner of the vehicle.
  • Oregon voter notification card;
  • Manufactured Structure Ownership documents issued only by Oregon Department of Consumer & Business Services;
  • Selective Service card;
  • Medical or health card;
  • Educational institution transcripts or other forms showing enrollment for the current school year;
    • Proof of Enrollment form is not acceptable
  • Unexpired professional license issued by an agency in the U.S.;
  • Form DS2019, Certificate of Eligibility for Exchange Visitor (J-1) status;
  • An approved letter from Oregon State Hospital, Veterans Administration Rehabilitation Clinics, Assisted Care Facility/home, Adult Assisted Living Facility/home, homeless shelters, transitional service providers, or group/half way houses certifying the customer's residence address
  • Mail from:
    • U.S. Treasury;
    • Social Security Administration;
    • U.S. Government agencies (including state, city and county agencies)
      • Mail from DMV can only be addressed to the residence address (mail from DMV containing a mailing address is not acceptable);
      • Mail from the United States Postal Service confirming an address change is not acceptable.
    • Utility companies;
    • Insurance companies;
    • State and Federal Revenue Departments;
    • Originators of out-of-state clearance letters; and
    • Educational institutions.
Your residence address is the actual address where you physically reside.  It is very important that you provide a good address when applying for a driving privilege or identification card.  Failing to provide a good address will cause you to not receive your permanent driver license, driver permit or identification card.  A mailing address may be used in addition to a residence address, but never in place of.
 
If you have an out-of-state residence and/or mailing address you will be required to provide proof that you are a resident of or domiciled in Oregon.  You will be required to complete a Certification of Oregon Residency or Domicile form and provide acceptable proof.  Proof for residency and/or domicile is noted on the back of the form.
 
Important: Mail must include applicants first and last name and cannot be addressed "in care of", "for", or "parent of".  Mail addressed with a “forwarding label” or an “address label” affixed to the envelope or contents are not acceptable. Mail must be dated within 60 days of application.  

Important: If you provide DMV with an address that is determined to be an address of a “mail service provider”, “false or fictitious” or an address known to be that of a “business”, you may be denied issuance or your privileges or card may be cancelled until you provide DMV with the address at which you physically reside.
 
All documents submitted must be acceptable to DMV.  DMV has the discretion to reject or to require additional evidence to verify your residence address.
 
Homeless
Individuals that are homeless may use a descriptive address such as "under the west end of the Burnside Bridge." Applicants needing to use a descriptive address must provide a mailing address and proof that he or she is a resident of or domiciled in Oregon.
 
You will be required to complete a Certification of Oregon Residency or Domicile form and submit acceptable documents at the time of application. Acceptable proofs of residency and/or domicile are noted on the back of the form.
 
Continuous Traveler
An applicant who travels continuously may use a residence address of "continuous traveler" if the applicant is a resident of or domiciled in Oregon but has no physical home in Oregon. For instance, an applicant who has retired from an Oregon employer may present papers indicating he/she sold a home in Oregon and purchased a motor home.
 
Applicants wanting to use a descriptive address as “continuous traveler” are required to provide a mailing address and prove that he or she is a resident of or domiciled in Oregon by completing a Certification of Oregon Residency or Domicile form and submitting acceptable proof. Acceptable proofs of residency and/or domicile are noted on the back of the form.
 
Military Personnel
