As part of the NAEP assessment, a series of questionnaires is administered to students, teachers, and school administrators to gather background information regarding student, teacher, and school characteristics; instructional practices and curricula; and information about students with disabilities (SD) or limited English proficiency (LEP). Similar to the development of the cognitive items, the development of the teacher, school, and SD/LEP questionnaire items is a consensual process that involves staff work, field-testing, and reviews by external advisory groups as well as reviews to ensure fairness and quality.

Whereas the student questionnaires are included in the student booklets, the questionnaires for teachers and school administrators are individual booklets.
