This SD/LEP questionnaire, which is completed by someone at the school knowledgeable about the student, asks about the student's background, the special programs in which the student participates, and the reason for the SD/LEP classification. This questionnaire is completed for each SD, LEP, or SD/LEP student in the sample, whether or not that student is included in the assessment.
