- Select EPO provides access to a national provider network and plan
coverage for a variety of health care needs. Features
include 100% coverage of preventive care and, after copayment, protection
with out-of-pocket maximum.
- Options PPO provides access to a national provider network, plus option to
seek services from nonnetwork providers. Plan features include
coverage for annual preventive care; annual deductible; out-of-pocket
maximum; and prior notification required for services such as inpatient
surgery and hospitalization.
- Definity Health provides
a Personal Benefit Account to help meet your up-front expenses. Features
include a national provider network; no required referrals for specialists;
100% coverage for annual preventive care; and protection with out-of-pocket
maximum.
- Core New Mexico is a Blue Cross of California fee-for-service plan that offers worldwide coverage with no network or primary care physician restrictions. Once a $3,000 calendar-year deductible is met, 80% of costs are covered. Out-of-pocket maximum per person is $7,600.
- New Mexico does not recognize "common law" marriages; therefore, only a legal spouse or eligible domestic partner may be enrolled in LANS sponsored health and welfare plans.

SAVE MONEY ON HEALTH CARE by using a Doctor/Dentist office or facility that is in your Insurance Carrier’s network. By using a Doctor/Dentist office or facility that is In-Network (UHC, Blue Cross, or Delta Dental’s network) you can avoid charges not approved by your insurance carrier, as long as no other insurance company is paying on your bill.
Example:
If your In-Network Doctor or Dentist office charges $80.00 for a procedure, but the insurance provider only allows $50.00 for that procedure, then the $30.00 difference is unallowable and will be removed from your balance due.
How can you tell if your physician is In-Network? Go to the insurance provider's web site and utilize the “find physician or providers” link to verify. Just because an office accepts your insurance card does not mean that they are In-Network! Ultimately it is your responsibility to ensure that your doctor/dentist or facility is in the network. Take the time to find out and save yourself time and money!
