Licensing Application and Form Downloads
ALL Applications and Fees must be submitted to the nearest USCG Regional Exam Center for processing. Failure to follow these guidelines will delay the processing of your application.

The Coast Guard recently conducted a Regional Examination Center (REC) Action Workout (AWO). The purpose of the AWO was to identify process improvements to the licensing program. The need for revision of all licensing related forms, as well as creation of additional forms, was highlighted during the AWO. Errors made in filling out the previous edition of the forms were noted as a factor in the delay in processing of mariner’s applications. The forms below are the product of the AWO. REC employees, industry representatives and stakeholders worked closely with our customers to make the forms easier to complete. These are the most current forms available for use in licensing transactions.

Mariners who desire to send correspondence or applications to a Regional Examination Center, should use the correct address (available here) and ensure that a return address is shown on the envelope. Incorrectly addressed mail will result in delays in processing. If sending original or certified copies of documents, it is recommended that you use certified mail or another mail service that provides tracking capabilities. Originals will be returned to you after verification.
