If you have not received your license renewal form by 5/30/2008, please complete a renewal form available to you on the Board’s web site, sign it, and send it in to the Board with the appropriate fee or your credit card information. The license renewal form on our web site will not have your information preprinted on the form. Be sure to complete this form completely to insure timely processing of your license renewal. We do not send renewal forms via facsimile (FAX) machine.
