Self-Management of Asthma Workgroup
Many Oregonians with asthma and the people that care for them do not have access to materials that provide them with the knowledge and skills necessary to manage the disease. Many Oregonians are not aware that asthma is a serious, life-threatening disease that can be controlled.
- Living Well with Chronic Conditions Workshop
Living Well with Chronic Conditions (the Chronic Disease Self-Managment Program) is a six-week workshop that provides tools for living a healthy life with chronic health conditions, including diabetes, arthritis, asthma and heart disease. Through weekly sessions, the workshop provides support for continuing normal daily activities and dealing with the emotions that chronic conditions may bring about.
- Oregon Asthma Resource Bank
Asthma is a complicated disease that requires constant self-management in between visits to the doctor. While there are many materials out there to support asthma self-management, they are of varying quality and accuracy. In response to the need for easily accessible, reliable materials, a collaboration of healthcare providers from throughout the state came together to create the web-based Oregon Asthma Resource Bank (OARB). On OARB you will find free, print-ready, clinically accurate, low-literacy, reliable asthma education handouts and provider tools based on the National Asthma Education and Prevention Program’s (NAEPP) Guidelines for the Diagnosis and Management of Asthma and the Guide to Improving Asthma Care in Oregon
- Self-Management Resources Distribution Tracking Form
Self-management resources are available for people who have asthma; however ensuring the information reaches patients is often difficult. As part of a larger effort to determine how well we are distributing asthma self-management resources and supporting self-management efforts, the Oregon Asthma Program is requesting your participation in tracking the distribution of resources. Please download the Self-Management Resources Distribution Tracking form and complete the sections that are applicable to your efforts.

The Goal of the Self-Management of Asthma Workgroup is that Oregonians with asthma and their caregivers will have easy access to the information, skills and support they need to manage asthma.
