September 22, 1998: Submission for OMB Review: Comment Request for Reclearance of Expiring Information Collections

The Office of Personnel Management published three notices regarding submission to the Office of Management and Budget of requests for clearance of information collections under the Paperwork Reduction Act of 1995, as follows:
- SF
2809-1, Annuitant/OWCP Health Benefits Election Form, will be used by
Federal retirement systems other than the Civil Service Retirement
System (CSRS) and the Federal Employees Retirement System (FERS),
including the Foreign Service Retirement System and the Office of
Workers' Compensation Programs (OWCP), and certain former dependents of

- SF
2809, Federal Employees Health Benefits Election Form, is used by
Federal employees, certain separated former Federal employees, and
former dependents of Federal employees, to enroll for health insurance
coverage under the FEHB Program. [Federal Register Notice: PDF or text]
- Information and Instructions on Your Reconsideration Rights, RI 38-47,
outlines the procedures required to request reconsideration of an
initial OPM decision about Civil Service or Federal Employees
retirement, Retired Federal or Federal Employee Health Benefits
requests to enroll or change enrollment, or Federal Employees' Group

September 18, 1998: Submission for OMB Review; Comment Request for Reclearance of a
Revised Information Collection: Form RI 20-80
- Announces that the Office of Personnel Management (OPM) has submitted to the Office of Management and Budget a request for reclearance of a revised information collection. RI 20-80, Alternative Annuity Election, is used for
individuals who are eligible to elect whether to receive a reduced
annuity and a lump-sum payment equal to their retirement contributions

- The Federal Salary Council is correcting the notice published

September 10, 1998: Proposed Collection; Comment Request for Review of a Revised
Information Collection: Form DPRS-2809
- Anounces that the Office of Personnel Management (OPM) intends to submit to the Office of Management and Budget a request for review of a revised information collection. DPRS-2809, Request to Change FEHB Enrollment or to Receive Plan Brochures, is used by former spouses and Temporary Continuation of
Coverage recipients who are eligible to elect, cancel, or change health

September 3, 1998: Federal Employees Health Benefits Program, Medically Underserved Areas for 1999
- The Office of Personnel Management has completed its annual
calculation of the States that qualify as Medically Underserved Areas
under the Federal Employees Health Benefits (FEHB) Program for the
calendar year 1999. This is necessary to comply with a provision of
FEHB law that mandates special consideration for enrollees of certain
FEHB plans who receive covered health services in states with critical
