You must send by postal mail the following documents:

• Letter of Appointment – sent to you via email
• Terms of Appointment
• IRS W-9 Form
• Authorization Agreement For Electronic Transfer of Funds
• Faculty Information Form or Student Information Form
• DHS Media Release
• ORISE Media Release
• Travel Form

To protect your personal information, mark envelope, "To Be Opened By Addressee Only."
