Setting My Goals
To set goals that are right for you, think about what you
want to change and why, and what steps you can take to reach your goals. These
changes dont have to be big. Even small steps can make a difference in
your health! Also, think about who can help you and how youll reward
yourself for making progress.
Use this chart to set simple physical activity and healthy
eating goals with your health care provider. You can also ask your family or
friends to help.
