Records Managment

The Albuquerque Fire Department Records Management is responsible for providing the community with EMS and Fire Reports while ensuring that all reports released are in compliance with HIPAA and Confidentiality Requirements. Business hours are Monday-Friday from 8:00 a.m.-5:00 p.m., the main number is 505-764-6337 and the fax is 505-764-6360.

AFD Records is located on the North side of the building. Once you're inside, make a left to the Records Lobby for AFD and APD. If you're in need of an EMS or Fire report, simply pick up the phone(tan) located by the AFD Logo, and Diana (764-6334) or Lita (764-6333) will be happy to assist you.

EMS Reports (Medical Service Rendered)

  • Patient - requires photo identification.
  • Parent may pick up a report with birth certificate (of child) and picture ID (of parent.)
  • If deceased, a death certificate and proof of executor of the estate are required.
  • A medical release is required if an insurance company or attorney is requesting a report.

Fire Reports

  • Fire reports are public record, and anyone can request fire reports with the exception of a casualty report - this is subject to the same criteria as an EMS report.

Any of the following information is needed to process a request:

  • Address/Location of Incident
  • Date of incident
  • Incident number
  • Name of Person
  • Time of Incident
