National Science Bowl®

for Middle School Students

Please note that this link is only accessible to existing Regional Coordinator's who have already been provided with their account log-in information. 

Regional Site Registration Form - Register here if you are applying to become a new 2008 Regional Coordinator and host your own Regional Science Bowl.  Existing Regional Coordinators need to LOG IN as a Coordinator and update their Regional Site Registration Form from last year.

September 5, 2008 Regional Site Registration Forms are Due
January 17 - March 7, 2009 Regional Event Season (events must occur during these dates)
March 11, 2009 National Forms Deadline
April 30 – May 5, 2009 National Science Bowl Event
August 18 - 20, 2009 Coordinator Biannual Meeting, Location TBD

National Forms

Immediately following each of the regional events the following forms must be submitted for the national event:

Team Forms 

  • Oversee that your Regional Winning team submits All of their completed and signed forms to DOE Headquarters (address is below).

  • Review and approve the team bios.

Coordinator Forms Completed and Submitted On-Line

Coordinator Forms Completed, Signed and FedEx'd to DOE HQ

Please note that these forms are in a PDF form-filler format.  To fill out these forms, open the file, place your cursor on a line and type in the information. The “Tab” key will jump to the next field to be completed. When you have completed entering all of the required information, print out the page, add the necessary signatures and dates, and FedEx the form(s) to Cindy Musick.  If you close the file, your text will not be saved.  Note: You are asked to sign the student and other adult code of conduct forms.
