Exam Materials Online Request Form

This online form will allow you to request exam materials required in order to become a licensed Private Pesticide Applicator. Upon completion of this form, the Colorado Dept. of Agriculture will mail you the open-book examination. You must pass this examination in order to apply for licensure through the Colorado Dept. of Agriculture.
