Crotched Mountain Community Care Program

Provides support, advocacy and community connections to residents of New Hampshire, including caregivers, their family members and individuals who are seeking to achieve and maintain a safe and healthy lifestyle at home in their community. Care management services include arrangement for transportation to medical appointments as needed.

New Hampshire Bureau of Elderly & Adult Services

Provides a variety of supports to adults age 60 and older and to adults between the ages of 18 and 60 who have a chronic illness or disability. Services range from home care, transportation assistance and assisted living to nursing home care and legal support services.
