Instructions for Faxing Proof of Claims
You may now fax file a Proof of Claim to the court if you are a creditor (the person who is owed money in a bankruptcy case) and are not represented by an attorney (form below).
Please complete the fillable form and fax it to the court at 602-682-4004. You should also include additional attachments supporting your claim such as invoices.
The faxed claim will be held until reviewed by a court employee and, if accepted, will be considered filed as of the received date of the fax. A Proof of Claim cannot be accepted for filing if you do not reference the bankruptcy case number on the form. If you have received a pre-printed Proof of Claim form from the Court, please use it as it displays the debtor’s name as well as the case number.

Please note: If you are a registered ECF user, and file your claim electronically, please refer to the Claims Filer instructions below to file your Proof of Claim.
