The Excel spreadsheets found below represent data regarding TERMINATED RECORD activity in the 340B Drug Pricing Program. Included are:

All Terminations – count of terminated records from 1994 – present in Excel format
CY 2003 – records terminated in calendar year 2003 by entity type and terminations code in Excel format
CY 2004 – records terminated in calendar year 2004 by entity type and terminations code in Excel format
CY 2005 – records terminated in calendar year 2005 by entity type and terminations code in Excel format
CY 2006 – records terminated in calendar year 2006 by entity type and terminations code in Excel format
CY 2007 – records terminated in calendar year 2007 by entity type and terminations code in Excel format
CY 2008 – records terminated in calendar year 2008 by entity type and terminations code in Excel format
CY 2009 – records terminated in calendar year 2009 by entity type and terminations code in Excel format
