  WELCOME TO THE OFFICE OF PHARMACY AFFAIRS DATABASE
IMPORTANT NOTIFICATION
Office of Pharmacy Affairs (OPA) is operating under the normal schedule and deadlines. The quarterly deadlines for submission of registration forms to OPA are December 1 to become effective as a participating covered entity for the quarter beginning January 1; March 1 to become effective as a participating covered entity for the quarter beginning April 1; June 1 to become effective as a participating covered entity for the quarter beginning July 1; and September 1 to become effective as a participating covered entity for the quarter beginning October 1. If your submission is close to the deadline, you are advised to FAX the form to OPA and mail the original.

DATABASE ENHANCEMENT ANNOUNCEMENTS
The Office of Pharmacy Affairs will use this space to announce enhancements to the 340B database as they occur. If you have a suggestion for how we can further improve the database, please submit via the link above.

01/1/2009 – rearranged Grant/Provider number and Entity Type fields in the record view.

4/24/2008 – added online registration for FP covered entities.

2/20/2008 – added Start Date and Termination Date columns to the "Children Entities" and "Other Related Entities" displays.

EXISTING DATABASE FEATURES

MEDICAID PROVIDER INFORMATION – users may go to the Covered Entity tab and find a sub-tab labeled “Medicaid Provider Number Search” (http://opanet.hrsa.gov/opa/CE/CEMedicaidExtract.aspx) that will list Medicaid Pharmacy Provider Numbers for all covered entities within a specified State that have indicated to OPA that they are billing Medicaid for drugs purchased at 340B prices. In addition, the database now accepts multiple Medicaid Pharmacy Provider numbers for each covered entity and NPI numbers. Medicaid Exclusion Tutorial – gives guidance to the covered entities for what needs to be included in the Medicaid Provider Number Field.

DOWNLOAD FEATURE – users have the option of selecting specific database fields to download for covered entity information. Go to the Covered Entity tab and under the “Output Type” select “Report File.” An additional menu will open up. If the user selects “Customized Fields,” you will be able to select the specific fields you want to be downloaded in an Excel spreadsheet format. For those organizations with a need for the old Legacy Report that is no longer available, a similar report is obtained if you go to the Covered Entity tab (Covered Entity Data Extract screen), under Output Type select the Report File option, select Customized Fields, and do NOT select the button labeled "Include Billing, Shipping and Contract Pharmacy information in Enhanced Download."
