Care coordination has been defined as “a process that links CSHCN to services and resources in a coordinated effort to maximize the potential of children and provide them with optimal health care.”1 While care coordination plays an important role in the care received by CSHCN, not all families who need this coordination receive it. To measure the proportion of CSHCN receiving care coordination, the survey asked parents about their satisfaction with the communication among the child’s doctors and between the child’s doctor and other providers, and about how often care coordination was available, if needed.
