Please include the Laboratory Contact Person in the attention line. Test codes for laboratory examination requested.

**For non-typhoidal Salmonella, Shigella , and E. coli O157 isolates included in NARMS surveillance, the 1st isolate collected each year should be submitted, followed by every 20th isolate
