Update to Form I-905, Application for Authorization to Issue Certification for Health Care Workers. New Edition Dated 11/10/20.
Edition Date: 11/10/20. We will also accept the 10/30/11 and 02/11/14 editions. You can find the edition date at the bottom of the page on the form and instructions.
