Badge Office location

As the central point where badges are issued to employees and official visitors, the Badge Office is located on the second floor of the Otowi Building (TA-3, Bldg. 261).

Monday-Friday: 7:00 am – 3:30 pm
Wednesday: Closed 11:30 am - 1 pm
Friday: Closed 12:00 pm - 1 pm

Returning Expired Badges

Do not return badges to the Badge Office in person.

Use the drop box at the top of the ramp going into the Otowi Building.

Badge requirements

  • US citizen employees must present a photo ID and proof of US citizenship. See Security Smart on Proof of United States Citizenship for the Badge Office (pdf) and Badge Office ID Cheat Sheet (pdf).
  • Foreign national guests and employees must have an approved visit and present a valid passport, I-94 Arrival/Departure Record (when appropriate), and their respective immigration document (permanent resident card, Form DS-2019, Form I-20, Form I-797 (E-3, H-1B, TN, O-1 approval notice)), if applicable.
  • Official visitors are those who have been invited by a Laboratory or contractor host employee. It is the host's responsibility to take care of any necessary badges or access permission and arrange to meet guests at a designated place. If you have any questions or concerns, contact Institutional Visits and Events, the Badge Office, or your host.
