The purpose of the Partnership program is to demonstrate that multi-partner collaborations that address social determinants of health and have a nationwide or regional reach, focus or impact can efficiently and effectively do one of the following: (1) improve access to and utilization of care by racial and ethnic minority and/or disadvantaged populations; (2) increase the diversity of the health workforce through programs at the high school or undergraduate level that focus on racial and ethnic health disparities and health equity and which include mentoring as a core component; or (3) increase data availability and utilization of data that increases the knowledge base regarding health disparities and facilitates the development, implementation and assessment of health equity activities.
Partnership program projects begin July 2017 and end June 2022.
The State Partnership Initiative to Address Health Disparities (SPI) is intended to demonstrate that partnerships in which (a) state offices of minority health/health equity (or other similar state entity) and state health agencies, or (b) tribes and tribal health agencies/organizations play a significant role, can efficiently and effectively improve health outcomes in selected geographical hotspots and address health disparities that affect minorities and disadvantaged populations. This program supports state partnership initiatives to: (1) produce a Health Disparities Profile on one to three Healthy People 2020 leading health indicator (LHI) topics chosen by the grantee for geographical hotspots (in the grantee’s state or tribal community) selected by the grantee, using existing baseline measures; (2) implement community intervention activities in the geographical hotspots selected by the grantee, using strategies to improve health outcomes and address health disparities for the grantee’s chosen one to three LHI topics (using baseline data from the Health Disparities Profile); and (3) publish the results of the implementation project and/or produce scholarly articles or brief reports which (a) compare the results of the baseline data/measures to the outcomes/impact measures, and (b) address improvements in health outcomes and health disparities for the selected one to three LHI topics. SPI projects began August 1, 2015 through September 1, 2015 and will end July 31, 2020.
The purpose of the State Partnership Grant Program to Improve Minority Health is to demonstrate the effectiveness of strategic partnerships to improve the status of minority populations and eliminate disparities in at least one of the following health topics: access to healthcare, asthma, cancer, cardiovascular disease/stroke, immunizations, diabetes, HIV/AIDS, infant mortality/LBW, mental health and/or obesity.
This program seeks to facilitate the improvement of minority health and elimination of health disparities through the development of partnerships.
