To prepare for the assessment process used during the PMF application, applicants should thoroughly review the Assessment Preparation Guide, found at the bottom of the "Become a PMF\Assessment Process" webpage. The Guide is subject to change and applicants are encouraged to check frequently for any updates. The Guide is only available shortly before and during the annual PMF application period. |

| Applicant Fax Cover Sheet |
This cover sheet is only applicable during the application cycle and customized for the annual application; it will be posted prior to the launch of the annual application and removed shortly after the application closes. This cover sheet should only be used by applicants who cannot upload documentation during the application process or otherwise requested by the PMF Program Office. Applicants are highly encouraged to upload their documents as part of their application as this will expedite the process.
