How to Get Started for Health Care Professionals

If you're a health care professional, there are two ways you may access your own information in the National Practitioner Data Bank (NPDB):

  • Self-Query: Search for reports matching the information you enter in your self-query order form. Self-Queries are $4.00 per order.
  • Review Your Report: If you have received a notification letter from the NPDB stating that you have a report, use the information in the letter to sign in and view your report.

Note: Information in the NPDB is not available to the public.
