Requesting Personnel Investigations via e-QIP


The “Requesting DCSA Personnel Investigations” portion of the website provides information on procedures for requesting background investigations from the Defense Counterintelligence and Security Agency (DCSA) to assist agency human resource and security offices. 

DCSA's Federal Investigations Processing Center (FIPC) receives requests for investigation, processes the requests through automated systems, and serves as an entrance point for case-related and operational questions. Agencies may contact FIPC for assistance in ordering investigations, obtaining access to DCSA's automated systems, or for information about DCSA investigative policies and procedures.

Submitting Office Number (SON)
DCSA assigns a unique four-character alphanumeric code, known as the Submitting Office Number (SON), to each office that requests investigations from DCSA. The SON identifies the office that initiates the investigation and is recorded in the appropriate Agency Use Block (AUB) of the SF 85, SF 85P, SF 85P-S, and SF 86. To obtain an SON from DCSA, complete a PIPS 12 form obtainable from the DCSA website or the FIPC Telephone Liaison at 724-794-5228. Agencies are required to keep the contact information for their SON current and accurate.

Submitting offices may have multiple SONs. Everyone authorized by the head of the Submitting Office can use the SON to obtain information on the case status of a background investigation from DCSA, provided the caller can answer the questions asked by Telephone Liaison. Security/Suitability Investigations Index (SII) data is not available to the SON.

Security Office Identifier (SOI)
An agency's Security Office is responsible for receiving completed investigation reports from DCSA, controlling the agency's cases, and making the suitability and security determinations on individuals being investigated for employment. Each Security Office is issued a unique alphanumeric four-character identifier from DCSA, the Security Office Identifier (SOI), which is used to identify the appropriate agency official who will receive case results, data, or other information from DCSA. Security offices are responsible for designating security office employees (authorized contacts) who may contact DCSA to obtain SII data (detailed information about a case). Only these "Authorized Contacts" approved by DCSA may obtain detailed case information. The authorized contacts must be able to answer identification questions posed by Telephone Liaison before information will be provided.

Each Security Office is responsible for providing DCSA with an official mailing address, contact information, and an approved list of employees authorized to request information about agency cases in process. The SOI should update this list regularly. Approved employees are the only individuals who may receive information by telephone. Contact Telephone Liaison at (724) 794-5228 or go to the DCSA website to obtain the PIPS 11 form required to obtain an SOI and update the SOI contact information. When submitting investigation requests to DCSA, the SOI may also be used in the SON block of the standard form if the SOI and SON are the same office.

Agency security officers must have a favorable determination based on the results of, at a minimum, a Tier 3 or Tier 4. All persons assigned adjudicative responsibility must be familiar with the laws and regulations governing suitability adjudications. Each Adjudicator must have been subject to a favorable determination based on the results of, at a minimum, a Tier 3 or Tier 5, in accordance with the results of the Position Designation System for Adjudicator's position. It is recommended that at least one or two individuals in an Adjudications Office have had a Tier 5. This is to ensure that in the event that classified material at the Top Secret level is included in a file, this material can be handled in accordance with current laws and regulations. For SII access, the minimum investigation level acceptable is Tier 3 or Tier 4.

Preparing to Request an Investigation

Position Designation
Investigation type, standard form type, and additional documents DCSA requires to conduct an investigation stem from position designation. The position designation is derived from the designation of a position’s assessed risk and national security sensitivity. Correct position designation is essential to the standardization of the investigation request process and reciprocity.

Using a position description and the hardcopy Position Designation System or Automated Tool, will provide you with the investigation type and form type needed for that specific position. However, it is important to note that accurate position designation requires precise and up-to-date position descriptions.

Position designation guidelines were initially created by OPM and implemented by the Federal Government in 2008 in order to support security and suitability reform efforts originating from the Intelligence Reform and Terrorism Prevention Act of 2004 (IRTPA). Validate Need
Validating the need for an investigation is the act of properly designating a position and practicing reciprocity as introduced in E.O. 13467 and subsequently outlined in E.O. 13488FIN 9-06 , FIN 10-04 , FIN 11-04FIN 12-01 , and  5 CFR 731 and 5 CFR part 1400. Before an investigation is requested from DCSA the requestor must check the Central Verification System (CVS) to see if a previous investigation meets the current position's requirements. 

While initiating an individual’s e-QIP request, an agency can utilize the validate need function within e-QIP to do a general check of CVS, which will check to see if there are any closed investigations within the last 5 years that meet or exceed what is being requested. Additional information on this website outlines the forms and documents required by DCSA to request an investigation. The forms are submitted after the position has been designated with the correct risk and sensitivity level and the need for an investigation has been validated.

Correction Requests
In order to avoid delays in processing investigation requests, it is important to verify the correct PII is reflected in DCSA’s systems of record. If a SAC (Case Type 92) was submitted with incorrect PII, submit documentation to validate the subject’s PII as part of the new e-QIP submission or request a correction for the SAC prior to submitting your investigation request. Send a message in NP2 to (S) FISCTR Boyers Corrections Tech (NTC), call 724-794-5612 x4414, or fax 724-794-1459. Provide one of the following data points:

  • Case or Request Number
  • Social Security Number
  • Subject’s first and last name

Identify the requested correction and the reason for the change. In addition, provide documentation to support the correction.

When submitting forms for processing, DCSA requests that agencies ensure the date fields are written as MM/DD/2020, and avoid abbreviating as MM/DD/20.

Once the investigation type and form type are determined, other investigative documents may be necessary depending on whether the investigation is an initial or reinvestigation, and whether the individual needing the investigation is a Federal civilian, contractor, or military. Charts on this page outline how these factors affect the submission requirements for fingerprint charts, the OF 306, and résumés.  The types of investigations DCSA offers along with the numeric codes used in the Agency Use Block (AUB) of the Standard Form are as follows:

Requesting Tier 2 Investigative Products without a Subject Interview
The Security and Suitability/Credentialing Executive Agents (SSC EAs), by memorandum dated June 5, 2018, have revised the 2012 Federal Investigative Standards to delete the Optional Enhanced Subject Interview (ESI) for Tier 2 investigative products.  Agencies must collect subject responses to the questions in the Additional Questions for Moderate Risk Positions form in addition to the Standard Form 85P prior to agency review and submission of the investigative package.  The Additional Questions for Moderate Risk Positions form must be submitted as an attachment to the investigative package.

Case types "T2S" (57) and "T2RS" (58) will continue to be utilized within electronic Questionnaires for Investigations Processing (e-QIP), until July 31, 2020. These case types will be in the Central Verification System (CVS), other DCSA systems and in completed investigations until such time as the systems can be updated.  The T2S and T2RS will be relabeled in DCSA guidance as simply Tier 2 and Tier 2 Reinvestigation. 

Requesting agency must continue to collect the information listed on the document titled "Additional Questions for Moderate Risk Positions" from the subject on case types "T2S"(57) and "T2RS" initiated on 1995 version of the SF85P. The document, available for agency use, is posted in the NP2 public library in the 2012 Federal Investigative Standards folder or you may request it from your Agency Liaison.

  • All questions on the document must be answered.
  • Document must be signed and dated by the subject of investigation. Documents may be digitally signed.
  • The document with the required additional information must be attached to the e-QIP request as an "ATS".

The e-QIP request will be rejected back to the requesting agency if the additional document is not attached as an ATS.  In this instance, the agency does not have to reject the request back to the applicant, but will have to obtain the additional subject responses and attach them to the e-QIP request before releasing back to DCSA.

For a limited period of time, agencies ordering case types T2S (57) and T2RS (58) will be charged the full case types pricing.  A billing adjustment will follow using an "EC" event code that credits the agency the equivalent TESI price if the interview is found to not be necessary after Standard form review.  If at any point during the investigative process an EFI trigger is presented in the case, a TESI will be scheduled on the case and the agency will see an "EC" event code notating the additional cost, i.e., reversing the previous credit.

Optional Form (OF) 306 "Declaration for Federal Employment"
The OF 306 is a form utilized to collect information during the hiring process to determine an individual's acceptability for Federal employment and Federal contract employment. It is also used to determine an appointee's status in the Government's Life Insurance program when the appointee has been previously employed by the Federal Government. Although the OF 306 is an optional form, providing the specific information requested by the OF 306 is not optional when OF 306 information is required for the investigation submission per the above tables.

COMPLETE OF 306 information must accompany each request for investigation submitted on an SF 85 when the individual to be investigated is a new Federal civilian employee or applicant for Federal employment. For SF 85P and 86 submissions, the OF 306 information should be submitted for such individuals only when there is a discrepancy between information submitted on the standard form and the OF 306 information. The individual and agency should carefully review the OF 306 information and any attached sheets. If additional space is needed, the individual may provide material on additional sheets. Any amendments to the OF 306 will require the individual being investigated to initial and date the amendments.

When submitting forms for processing, DCSA requests that agencies ensure the date fields are written as MM/DD/2020, and avoid abbreviating as MM/DD/20.

Investigations on contractor personnel requested on the SF 85 require the applicant to answer specific questions found on the OF 306: 1, 8, 9, 10, 11, 12, 13, 16, and 17a. Question 17a requires a signature and date in order for the form to be complete. To provide the additional information to DCSA, the OF 306 may be used, or the specific questions and answers may be provided on an attachment. The latest version of the OF 306 can be found here.

Application or Resume
An application or résumé should only be submitted to DCSA when there is a substantive discrepancy between the application/résumé and the Standard Form, and your agency needs that discrepancy addressed during the investigation.

Agencies sending the résumé to DCSA for review and scheduling must use FIPC Code “1B” in the AUB block of the Standard Form. Additionally, complete information for the residence, employment or education which the agency wishes DCSA to address will be required before DCSA schedules the investigation. To avoid delays in scheduling provide any needed supplemental information for the application/résumé on a separate document as an “ATA” attachment within e-QIP. 

As agency submitting offices are responsible for comparing the application/résumé to the Standard Form, applications/résumés provided with investigation requests without FIPC Code “1B” on the AUB will not be reviewed by DCSA. However the applications or résumés will be provided with the investigation results to the SOI for adjudicative purposes with a comment alerting the SOI that the application/résumé was not requested to be reviewed by DCSA.

Form Requirements for Foreign Coverage
If an applicant lived in one of the following Countries for 180 days or more, regardless of case type or form type, a release is required to be submitted as an attachment along with their investigation request in e-QIP: Ireland, Iceland, Argentina, or Australia. For the Ireland release only, a photo copy of page 1 of the applicant’s passport must be submitted along with the Ireland release form.

Standard Forms: Additional Information
All information requested on Standard Form (SF) questionnaires is essential for the expeditious investigation scheduling and processing. The Submitting Office (SON) is responsible for ensuring completeness of the SF 85, SF 85P, SF 85P-S, and SF 86. The agency must have an individual complete all information required, as DCSA will not accept incomplete investigation requests. Refer to table 2 for the correct investigation types to be used with each form. Automated case scheduling is based on zip codes and other data provided by the individual. Consequently, complete and accurate information on the investigative forms is extremely important to the overall investigative process. Incorrect zip codes, missing addresses, or gaps in dates may cause delays or rejection of the request.

Standard Form (SF) 85P-S, "Supplemental Questionnaire for Selected Positions"
The SF 85P-S is used only as an exception to standards. It contains additional questions regarding drug use, alcohol use, and mental health treatment that may be required for certain specified positions. Agencies with unique position requirements must request approval from DCSA to use this form. If you are uncertain if your agency has the required approval, or whether this form is appropriate for a particular position, contact DCSA at 724-794-5612.

Third Party Data Entry (3PDE)
DCSA does not accept hardcopy Standard Forms. DCSA recognizes that there are instances in which individuals may not be able to enter information directly into their e-QIP forms. To fulfill this need, DCSA created Fillable versions of the Standard Forms for use in the Third Party Data Entry process. 3PDE allows an authorized Agency e-QIP User to initiate a request in e-QIP and enter the individual's provided standard form information giving the requesting agencies the benefit of data validation in e-QIP.

An authorized e-QIP User must have the specific group role of "Third Party Entry" (TPE) in order to enter data in e-QIP on behalf of an individual. TPE agency users are presented with a disclaimer page of instructions at the beginning of the data entry. TPE agency users must acknowledge acceptance of these instructions before continuing with the data entry of the individual's standard form. Upon completion of data entry, the agency user will present the individual with the archival copy and e-QIP generated signature pages for review and signatures. Once signature pages are signed by the individual, agency users with the e-QIP role of "Reviewer" or "Approver" are required to upload signature pages to the e-QIP investigation request.

Agency users with the "Reviewer" and "Approver" roles in e-QIP are able to manage the completed third party data entered investigation request just as any other investigation request in e-QIP. The Agency Use Block (AUB) tab in e-QIP is still required to be completed accordingly by a TPE, Reviewer, or Approver in e-QIP.

Certification and Releases
A Certification and General Release are required with every investigation request. Standard Investigative forms such as the SF 85, SF 85P, SF 85P-S, or SF 86 must be submitted to DCSA within 120 days of the date that an individual electronically certified their Standard Form within the e-QIP system (not the date written on the Certification page if the individual signed their Certification page with pen and ink). Investigations rely on current information provided by the individual on the SF; therefore, investigation requests released to DCSA with certifications that were signed in e-QIP more than 120 days ago are deemed "unacceptable" for processing and returned to the requesting agency without action.

Certifications and releases printed from e-QIP contain the request ID of the questionnaire completed by the individual. The request ID printed on the Certification and releases must match the e-QIP investigation request ID being submitted to DCSA. The FCR is required to be submitted to DCSA with all investigations submitted on the 2017 version of the SF 85P and the SF SF 86. Information regarding acceptable electronic format of attachments and guidance on how to electronically attach these items may be found in the e-QIP Job aids maintained in the NP2 Secure Portal or through e-QIP help screens accessed via the help menus in e-QIP.

e-QIP has the ability to accept electronically imaged attachments as part of the investigation request. The imaged attachments include the signed investigation questionnaire Certification, releases, and special attachments. This does not include fingerprint charts. Please see fingerprints tab (link to fingerprints tab) for information about submission of fingerprints.

Submit the Medical Release, "Authorization for Release of Medical Information Pursuant to the Health Insurance Portability and Accountability Act (HIPAA)", with an investigation request to DCSA when the SF 86 contains a positive response to the mental health section (section/question 21 – Psychological and Emotional Health). An agency should also submit the Medical Release if there is a positive response to question 5 on the SF 85P-S. The request ID printed on the Medical Release must match the request ID of the e-QIP investigation submission. Reminder: Agencies must have a special agreement for use of the supplemental portion of the SF 85P, which is the SF 85P-S.

Click to Sign (Use of Digital Signatures within e-QIP)
Click to Sign provides individuals the ability to digitally "sign" their Certification and releases. The Click to Sign signature process is a separate process from digitally signing a document with Personal Identity Verification (PIV) Cards or Common Access Cards (CAC). This functionality is now available for the Certification page, Fair Credit Reporting Release, General Release and Medical Release. Applicants can still opt-out of signing their documents digitally and may still print, sign and either upload as a scanned attachment, upload via the fax-solution, or mail hardcopy to their sponsoring agency (sponsoring agency must electronically attach these hardcopy documents prior to releasing their request to DCSA). Information and instructions for the e-QIP Fax Solution can be found within the e-QIP Help-screens on the agency attachment screens. Special Attachments
Agencies may provide special attachments with the investigation request, such as license or certificate; issue information; individuals in the U.S. who can verify activities outside the United States; agency-conducted subject interview or pre-appointment checks; and any other pertinent information. To notify DCSA of special attachments provided with the submission, use Extra Coverage Code 7 when completing the Agency Use Block (AUB).

Form Requirements for Childcare Positions
Investigations for childcare positions using position code "H" (Tiers 2 through 5) or extra coverage code "8" (Tier 1) will often require documents for each state that the subject has listed residency since the age of 16. Document submission details for the states should be reviewed before submitting the investigation for childcare positions and can be found in the DCSA Childcare Agency Guide. This Guide is available in the NP2 Secure Portal in a public library folder labeled “Childcare Investigations Documents”. These documents are required in addition to the documents listed per government affiliation and case type on the “Case Types & Forms” pages.

Due to the state repositories needing original copies of fingerprint cards and other required documentation, all original copies of forms must be mailed to the address below. Special Agreement Checks (SACs)
SAC availability, codes, and pricing for the current fiscal year can be found on the DCSA Billing Rates. The INV Form 86C can be used to request a number of DCSA's National Agency Checks as Special Agreement Checks, case type 92. Fingerprint requests are also considered a SAC product. Please note: By submitting an investigative request using the INV Form 86C, the agency is acknowledging that the fees associated with the SAC request have been approved by the agency. Additionally, the Periodic Assessment SAC and National Crime Information Center/Interstate Identification Index SAC require Special Agreements with DCSA before an agency can begin requesting them. Contact DCSA at 724-794-5612 to initiate a special agreement.

Reimbursable Suitability/Security Investigation (RSI)
An RSI can be requested in order to expand on issues or to obtain additional coverage where appropriate. DCSA must also be in possession or receipt of a complete SF 85, SF 85P, SF 85P-S, or SF 86, including a signed Signature Form/Certification and a signed Authorization for Release of Information. For directions on how to submit RSI requests via the NP2 Portal using FIPC Form 554, please visit the DCSA Reopen and RSI webpage. Questions regarding the submission or pricing for the RSI product should be directed to DCSA Telephone Liaison at 724-794-5228.

Upgrade and Downgrade Requests
Upgrade/Downgrade/Case Service change requests are for pending investigations only (closed cases cannot be upgraded or downgraded). Requests must be submitted to DCSA's Correction Techs on agency letterhead via fax (724-794-1033 or 724-794-1459) or email (fisupgradedowngrade@opm.gov). Requests must contain the following information:

  • Agency Name
  • Agency Phone Number
  • Agency Authorized Requestor's Name
  • SOI or SON
  • Date that the Upgrade/Downgrade request is being made
  • Subject's Name
  • Case Number or e-QIP Request ID of the current pending investigation
  • Reason for Upgrade/Downgrade request
  • New case type requested and any changes that the new case type warrants within the Agency Use Block fields (i.e., Service Code, Access, Risk, Sensitivity)
  • Any other additional information needed to process the new investigation type

NOTE: You cannot upgrade to an investigation type that may require a new Standard Form (SF) type. For example you cannot upgrade an investigation that was submitted on an SF 85 to an investigation that requires an SF 86. In this instance you would need to discontinue the first investigation, have the applicant fill out the correct Standard Form within e-QIP and submit the new investigation request on the correct form.

Reimbursable Suitability/Security Investigations (RSIs) and Reopens
Reopen/RSI Status Questions can be directed to 724-794-5228. NBIB processes agency requests to reopen investigations and conduct Reimbursable Suitability/Security Investigations (RSIs). Previously closed investigations can be reopened for the following reasons:  clarification of issues; to obtain additional information within the scope of the investigation; to complete a Discontinued investigation or an investigation previously closed Incomplete.  An RSI can be requested in order to expand on issues or to obtain additional coverage where appropriate.

Agencies may submit their requests via the NP2 Secure Portal. If you have questions about the NP2 Secure Portal or questions on how to obtain an NP2 Secure Portal Account, please call Systems Access Liaison at 724-794-5612, extension 4600.

Reopen requests must be submitted on the FIPC 553. Additional guidance can be found on page 2 of the FIPC 553.

If the request is to reopen a Discontinued case, please provide:

  • Current Employer (name, address, city, state, zip, phone number)
  • Full physical work location
  • Dates of employment
  • Supervisor name and address

RSI requests must be submitted on the RSI Request Form - FIPC 554. (Additional guidance can be found on page 4 of the FIPC 554.)

Please note, if an agency requires only a National Agency Check, the cost to run these checks is significantly less in a Special Agreement Check (SAC) than an RSI. Therefore, if the type of check your agency needs is available as a SAC (see the SAC listing), you may want to request the SAC(s) via the hardcopy INV 86C or some SAC codes are available through the “Request a SAC” function in CVS.  See the Federal Investigative Notices for the most current Billing FIN and prices.

Expedites for Deployments (Catch'em in CONUS)
The purpose of the Catch'em in CONUS program is to attempt to complete an interview with the subject that is going to deploy, or a subject that has been deployed, but will be stateside on leave for a limited time and requires an interview. We make every effort to interview the subject while he/she is stateside.

To expedite a subject interview due to the individual deploying or returning from deployment for a limited time, email the request to FISCatcheminCONUS@nbib.gov. Please include the following information:

  • Subject's full name
  • NBIB case number, Social Security Number or e-QIP Request ID
  • Start and end date of subject's stateside availability
  • Stateside/leave location: Full address

Fingerprint Submissions
Fingerprint submissions are required for specific investigation types sent to DCSA. Fingerprint submission requirements are outlined in the charts on the Investigation Case Type & Forms tab. Always remember to select the applicable fingerprint submission type and method within the investigation request in e-QIP. This will ensure proper handling of the investigation request and fingerprints once they arrive electronically or hardcopy at DCSA.

The FBI retains all the fingerprint images sent by DCSA, which currently supports the Federal civilian and military post-appointment arrest information process. This process enables subsequent FBI criminal arrest information to be forwarded through DCSA to the employing agency of a Federal employee or military personnel. In such instances, DCSA furnishes the agency contact with a copy of the FBI record and a FIPC 402, Agency Adjudication on FBI Post Appointment Arrest Form, characterizing the issue(s). The information is recorded in the Security/Suitability Investigations Index (SII).

Fingerprint requests are sent separately to DCSA. Agencies are strongly encouraged to submit fingerprints electronically instead of mailing hard card charts. Fingerprint documents must be received before DCSA will process the e-QIP request. A credit may be applied to the higher level investigation in the amount of the initial fingerprint SAC only if the fingerprint is received at least 1 day, but no greater than 120 days, in advance of the higher level investigation. If an e-QIP request is received and the investigation type requires submission of fingerprints, and there is no evidence of a fingerprint result (result can be no more than 120 days old) on file with DCSA, the investigation request may be held for up to 14 days awaiting fingerprint documentation. If after 14 days the required documentation is not received, the e-QIP investigation request is flagged as unacceptable and returned to the submitting office. Automated daily notification reports are available to assist agencies with identifying e-QIP investigations in danger of being made unacceptable. Please see FIN 13-03 or contact DCSA’s System Liaison at (724) 794-5612, extension 4600, to receive daily notification reports.

It is imperative that identification of the person being fingerprinted is checked and that the identifiers entered electronically or written on the SF 87 or FD-258 are confirmed with the individual being fingerprinted before the fingerprints are electronically transmitted or mailed. The correction of erroneously submitted information delays timely completion of the investigation.

Electronic Submissions
DCSA recommends submission of electronic fingerprints through the Fingerprint Transaction System (FTS). Submission of electronic fingerprints through FTS improves efficiency, timeliness of results, and quality of the print. Electronic fingerprint submissions will be scheduled to the FBI as a Special Agreement Check (SAC), Case Type 92.  The SAC results will be furnished to the designated SOI as soon as the SAC is completed. These SAC results can be used to satisfy fingerprint requirements of a higher level investigation request for 120 days. Always use FIPC Code “I” in the Agency Use Block (AUB) of the e-QIP request if your agency is using an electronic fingerprint submission to satisfy a fingerprint requirement for that investigation request. The “I” code ensures the SAC results are linked to the higher level investigation and that billing adjustments occur correctly.

Electronic fingerprints are submitted to the FTS electronically via an FBI approved live scan system or fingerprint card scan system that utilizes FBI and DCSA-approved software. Currently, DCSA only accepts Type-4 fingerprint images for electronic submission. Visit the CJIS/FBI website to select from a complete listing of FBI-approved vendors.

Type-4 fingerprint images consist of the following:

1 Plain Left and Right Simultaneous Four Finger Impressions
1 Plain Left and Right Thumb Impression

Your agency is responsible for contacting your vendor for questions on configuring, installing, or testing your electronic fingerprint machines. Refer to FIN 00-04 for information on machine configurations.

Hardcopy Submissions – SF 87 and FD-258
DCSA recommends submission of electronic fingerprints through FTS as it improves efficiency, timeliness of results, and quality of prints.  When electronic submissions are not feasible, DCSA will accept a hard card submission using the December 2017 Standard Form 87 (SF87) for Federal employee/applicant, military, volunteers as well as contractors.  Until further notice, DCSA will continue to accept the FD-258; however, the December 2017 SF 87 may now be used for contractors since the FBI is retaining all prints regardless of which card (form) type is used to submit.

SF 87’s will automatically schedule as a SAC, case type 92 because the SOI, SON and IPAC fields are required. FD-258’s will schedule as a SAC, case type 92 when the SOI, SON and IPAC are provided in the “Reason Fingerprinted” field. If the SOI, SON and IPAC are not provided on the FD-258 and there is no previously received investigation request at DCSA to which a fingerprint chart can be matched, the fingerprint chart will be held for 30 days until it gets mailed back to the return address on the original mailing envelope.

When submitting hard card fingerprints to satisfy fingerprint requirements of a higher level investigation, the hard card fingerprints must be received by DCSA within 14 days from when the e-QIP investigation request was released to DCSA.  If the hard card fingerprints are not received, the e-QIP request will be returned to the agency as “Unacceptable”. When sending hard card fingerprints, it is recommended that a coversheet be included with the name and contact information of the agency official sending the fingerprint, subject identifiers and, if applicable, the e-QIP request ID. A coversheet with an FD-258 should also include the SON/SOI/IPAC of the submitting agency.

Ordering Fingerprint Cards
As of October 1, 2014, OPM no longer stocks the SF 87 cards. Agencies can purchase SF 87 (stock #7540-00-634-4037) from the General Services Administration (GSA) using their own GSA account.  The FD-258 is owned by the FBI. To order more FD 258s, please visit the FBI’s website.

Agencies should fill out the top portion of the SF 87 and FD-258 as indicated below:

*NOTE: If an agency wants fingerprints on an FD-258 to schedule as a SAC, case type 92 when DCSA receives the hard card, the SOI, SON and IPAC should be written in the “Reason Fingerprinted Field”.

FIPC Codes Regarding Fingerprint Submissions
FIPC Codes go in the FIPC Code field in the Agency Use Block on the Standard Form within e-QIP. They do not go on the fingerprint charts.

R Advance Fingerprint results report is required by agency. (See FIN 06-04 (PDF file) [777.17 KB]) This code will trigger an Advanced Fingerprint Report to be generated the day after completion of the criminal history check. That report is then immediately sent to the SOI in whatever method your agency currently receives investigative results. NOTE: If you use FIPC Code R and your fingerprints were scheduled as a SAC (case type 92 for the same SOI), the same fingerprint results will be sent to the SOI multiple times.

 

Requirements for Submitting Reprints when Prints are Unclassifiable
When the FBI is unable to make a classifiable determination because a search of the FBI criminal history records cannot be completed due to illegible fingerprints, the FBI deems the submissions as “Unclassifiable”. A reprint may be submitted within one year of the case closing date in an effort to obtain a classifiable determination without incurring additional cost. If it is required that the reprint be included in the prior closed investigation, the reopen request must be submitted within one year of the case closing date and within 120 days of the reprint results.

A second submission may be sent electronically or via hard card regardless of how the first print was submitted. The original case number of the unclassifiable fingerprint must be provided with the reprint submission to avoid the discontinuance of the reprint or an additional fingerprint processing charge. This case number can be found on the Case Closing Transmittal (CCT). The original case number must be provided in the OCA block on the SF87 or FD-258 hard card or in the tagged field 2.009 of the electronic transmission. When submitting a hard card, please note “Reprint” in the top left-hand corner and attach the Notice of Unclassifiable Fingerprints. All reprint results will be updated in PIPS and a new CCT will be provided to the agency identifying the results of the fingerprint submission as a “Reprint”.

For electronic submission, enter original case number in tagged field 2.009. The field name will vary based on vendor.

For hardcopy submissions, enter the original case number in the Originating Case Agency (OCA) number field on the SF 87 or FD-258. Submit the hardcopy card with "Reprint" written in the top left hand corner, and provide the Unclassifiable Notice indicating the original fingerprint was unclassifiable.

Fingerprint Special Agreement Check (SAC)
A fingerprint only Special Agreement Check (SAC) may be conducted prior to submission of any investigative request and may be captured either electronically, using a Live-Scan System or Fingerprint Card Scan System (and then transmitted to OPM's Fingerprinting Transaction System (FTS) or captured hardcopy (and then mailed to OPM). The fingerprint request is processed as a SAC case type 92 for both submission types. For a hardcopy case type 92, clearly identify the SON/SOI/IPAC codes. For SF 87, the SON/SOI/IPAC information should be listed in the Department, Bureau, and Duty Station block.  For FD-258, it should be listed in the Reason Fingerprinted block. Either method provides the customer agency with timely results to fingerprint searches.

Special Notes on Fingerprint SAC Requests
If a fingerprint SAC is submitted in advance of the higher level investigation, a credit may be applied to the higher level case in the amount of the initial fingerprint SAC.

To receive this credit, the requester must:

  1. Submit the fingerprint SAC at least 1 day, but no greater than 120 days, in advance of the higher level investigation, and
  2. Include FIPC Code “I” (letter i) on the higher level investigation.

 

Fingerprint Submission Requirements when No Prints Can be Obtained from Subject
An electronic fingerprint submission must account for all Type-4 fingerprint images to complete a Fingerprint SAC.  When some fingerprint images cannot be obtained due to injury, amputation, deformity, etc., your vendor’s software should allow for coding in data field 2.084 to account for the fingerprint that is missing.
Code “XX” can be used to account for a finger that is physically missing due to amputation, missing at birth, severed, etc.
Code “UP” can be used for a finger that is physically present but unable to print due to injury, burn, deformity, bandage, etc.
NOTE:  If your vendor’s software does not allow for such coding, you must contact your vendor.

A hard card fingerprint submission using an SF 87 or FD-258 must also account for all Type-4 fingerprints to complete a Fingerprint SAC.  The SF 87 or FD-258 must be documented to reflect fingerprints that cannot be obtained by using the same “XX” and “UP” codes.

A Fingerprint SAC cannot be scheduled from an electronic or hard card fingerprint submission when no Type-4 fingerprint images can be obtained due to injury, amputation, deformity, etc.  Do not submit an electronic fingerprint submission with all image blocks reflecting “XX” and/or “UP” codes.

Fingerprint submission requirements may be met under these circumstances by requesting an FBI CJIS Name Check (FBFN item).   The request may be submitted without the SF 87 or FD-258 if the request includes all information requested on the top portion of the SF 87 or FD-258 along with the SON, SOI, IPAC, and the reason the fingerprints cannot be obtained.  The request may be sent to DCSA via the NP2 Secure Portal at “(S) FISCTR Boyers Corrections Tech (NTC)”.

The following template may be used to provide required information for FBFN item scheduling:

The Intra-Governmental Payment and Collection (IPAC) Agency Location Code (ALC) is assigned to your agency by the U.S. Department of Treasury. The IPAC System provides a standardized interagency fund transfer mechanism for Federal Program Agencies (FPAs). IPAC facilitates the intra-governmental transfer of funds, with descriptive data from one FPA to another.
This code is required for all investigation requests. The code IPAC is also commonly referred to as the ALC.  For DCSA customers that will not make payments to DCSA within the IPAC system a DCSA designated agency code will be provided for use within this field.

Beginning January 1, 2014, Treasury required that the TAS only be provided in the component format. The Component TAS is a financial code, assigned by the Department of the Treasury, comprised of many separate, component pieces/ sub-fields* totaling 27 alphanumeric characters; however, not all sub-fields apply to each fund account. Among other things, the financial code identifies the agency, the period of availability of funds, and a specific fund account. This is the document number of the purchase order or other obligating documents prepared by the requesting agency. It is usually a 17-digit alphanumeric code. This number is usually assigned by the agency’s contracting office. It may change annually, or more frequently, depending on how much an agency obligates for investigations. Once the obligated money runs out, a new obligating document or modification is needed.

This can be up to an eight-digit alphanumeric code that indicates the type of activity being reported (payments, collections, etc). Some agencies are using DISB if receiving services and COLL if performing services. BETC determines the transaction effect on the Treasury Account Symbol’s Fund Balance with Treasury. BETC replaces transaction codes and subclasses, but at a more detailed level.

Accounting Data and/or Agency Case Number (Optional field)

The accounting data field on the Agency Use Block (AUB) is an optional field (25 character limit) that can be populated on the standard form. This block can be used by the requesting agency to note any information that the agency needs for its own internal processes, such as an internal case number or any other specific agency identifier.  This field will appear on your monthly invoice and can be useful for internal invoice reconciliation.

There are two options, select only one:
Initial: Select if the investigation request is an initial investigation.
Reinvestigation: Select if the investigation request is a reinvestigation.
NOTE: The selection of the "initial" or "reinvestigation" check box must align with the type of investigation requested. Do not request an initial investigation type (i.e. Tier 3) and then select the reinvestigation check box. This will delay the scheduling of your investigation.

Requesting Official (Required field)

Enter the name and title of the official requesting the investigation; enter the commercial telephone number, including area code. A valid email address is highly recommended, but not required.  Remember to keep contact information current in AUB templates to avoid delays and discontinue of investigations.

The individual listed in this field may be contacted by DCSA’s scheduling team, field investigators, billing department, and/or case review team to answer questions about the investigation request.  If your agency has provided a group email address on the PIPS Form 11 or PIPS Form 12, the scheduling team will make contact to this address before reaching out to the requesting official.

Secondary Requesting Official (Optional field)
Standard Form 86
Enter the name and title of the secondary official requesting the investigation; enter the commercial telephone number, including area code.  A valid email address is highly recommended, but not required.  Remember to keep contact information current in AUB templates to avoid delays and discontinue of investigations.
The individual listed in this field may be contacted by DCSA’s scheduling team, field investigators, billing department, and/or case review team to answer questions about the investigation request.  The secondary requesting official will be contacted if attempts failed for the group email address and/or primary requesting official.

Processor (Optional field)
Standard Form 85, 85P
The Processor block reflects the individual who has initiated, reviewed, or approved the request within e-QIP. If not utilized, click the “Copy From Requesting Official” button.

Agency Special Instructions for the Investigative Service Provider (Optional field)

If the agency has any special handling instructions for the investigation being requested, they are to be completed in this section. (This is to be used in place of memos of instruction.)

Enter the affiliation of the position for which the investigation is being requested. There are four options. Select one from the drop-down menu:
FED CIV: Federal Civilian position
MIL: Military position
CON: Contractor position
Other: Positions not designated as Federal civilian, military, or contractor

Deployment/PCS (Optional field)

Use this field if the individual being investigated will be deployed or assigned overseas during the processing of the individual’s background investigation. In addition, the DCSA recommends agencies reach out to the Catch’ em in CONUS team.

The purpose of the Catch’em in CONUS program is to complete an interview for a subject that is going to deploy or a subject that has been deployed but will be stateside on leave for a limited time and requires an interview. The DCSA makes every effort to interview the subject while he/she is stateside; however, we cannot guarantee anything.

Over the years, the DCSA has noticed that if we receive notification too far in advance, numerous events can affect our success in interviewing your employee.  These events include; change in availability, trip cancelled or postponed, change in R/R location or the CAF grants eligibility.  Please send availability information no more than 30-45 days from subject’s arrival in the States.

A CAGE code is a unique 5 character identifier for entities doing, or wishing to do business, with the Federal Government. The format and character position of the code may vary based on country.

Contracting Number (Optional field)
The Contracting Number is limited to 50 alphanumeric characters.

Extra Coverage Codes
Extra coverage codes are used to request information or processing beyond the normal scope of the investigation. Certain codes result in an additional charge. For a list of current investigation products and services offered by DCSA, refer to the billing rates Federal Investigations Notice for the current fiscal year.

NOTE: Codes marked with an X require a special agreement with DCSA or contingency factors apply.

Extra Coverage Code Description By Special Agreement or Contingency Factors Apply
1 Overseas attributes (special questioning during the Enhanced Subject Interview required). Can only be used with T4, T4R, T5 and T5R investigation types.  
2 To comply with the 2012 Federal Investigative Standards, a credit search is now required on all T1 investigations if the subject admits to Federal debt or if it is discovered during the investigation. This extra coverage code is used to request a Credit search with the NAC or T1, whether Federal debt is known or unknown at time of scheduling the investigation. An additional fee is charged for this optional credit check. (Agency must maintain Fair Credit Release, unless submitted on the 2017 version of the SF 85P or the SF 86, which includes a Fair Credit Release to be submitted with the investigation.)  
3 Advance NAC: The advance National Agency Check (NAC) consists of an itemized list of the NAC results and search status. This is a notification of item results only; no hardcopy is furnished. The Advance NAC Report is sent to the SOI and is available for all case types. If after 30 days from the scheduling date the NAC is not complete, a NAC status report will be provided to the SOI.  
6 Law Enforcement attributes (special questioning during the Enhanced Subject Interview required). Can only be used with T4, T4R, T5 and T5R investigation types.  
7 Special Attachments: This code is used when an agency wishes to call special attention to attachments provided with the submission, such as request for license or certificate verification; issue information; personnel folder or security file information; people in the U.S. who can verify activities outside the United States; agency-conducted subject interview or pre-appointment checks; and any other pertinent information. It is available for use with all case types.  
8 Child Care T1 request: Statewide criminal history background searches on individuals who are seeking a position, which involves having direct contact with children, per the Crime Control Act of 1990—Public Law 101647. An additional fee is charged for these searches.  
A Investigator Positions. Can only be used with the T4, T4R, T5 and T5R. X
B Astronaut Positions. Can only be used with the T4, T4R, T5 and T5R. X
CC Catch 'Em in Conus. Can be used on all case types, except the NAC and T1. X
CL Catch 'Em Linguists. Can be used on all investigation types except the NAC and T1. X
CT Catch 'Em in Training. Can be used on all investigation types except the NAC and T1. X
H Used to request Citizenship and Immigration Services (CIS) check to verify the individual’s immigration status if not a U.S. Citizen or citizenship status if a naturalized U.S. Citizen. Available with the NAC.  
J Selective Service will schedule automatically on all investigations where the subject indicates they were born a male after December 31, 1959. If this check will not be automatically scheduled, but will still be required for the investigation it can be requested using extra coverage code “J” on all investigation types.  
L Bureau of Vital Statistics (BVS): Used to request verification of birth record information on file at listed State of Birth. Available for all investigation types.  
Q Bar Association License Checks: Used to request verification of state bar membership for attorney positions. Available for use on all case types except those that use the Standard Form (SF) 85 (T1 and NAC on an SF 85).  
S Advance Verbal results of subject interview. Available with investigation types: T4, T4R, T5 and T5R.  
W Military checks. Only available with the NAC. These checks occur automatically on all investigation types except the NAC.  
WW Wounded Warrior: For military applicants who are permanently disabled due to combat-related injuries. This code sends investigation through specific review processes at DCSA before completion. Available for use with all investigation types except the NAC.  
Z Criminal Justice Position – FBI user fee exemption: The FBI charges user fees for processing Federal fingerprint and name search requests that are not specifically for criminal justice or law enforcement purposes. Positions eligible for exemption of the FBI user fee must support the agency’s criminal justice mission. According to the FBI in 42 U.S.C. 14616, Article V(6) , criminal justice includes activities relating to the detection, apprehension, detention, pre-trial release, post-trial release, prosecution, adjudication, correctional supervision, or rehabilitation of accused persons or criminal offenders. The administration of criminal justice includes criminal identification activities and the collection, storage, and dissemination of criminal history records. The extra coverage code "Z" is to be utilized on those positions exempt from the FBI user fee. For additional information regarding the FBI user fee exemptions, please see FIN 14-04. FIPC Codes
FIPC Code indicates special processing needs for a particular request. These codes can be selected in the drop down menu in e-QIP.

1B Used to indicate to DCSA that the agency requires review and scheduling of information from the resume. Available with T1, T2, T3, T4, T5 and NAC. See section 5.5 for more information.

4 STATE PASSPORT FORM 240 Birth Verification Required. Available with NAC only.  
I Fingerprint results on file with DCSA as a SAC, case type 92 completed within 120 days prior to the submission of the e-QIP request. Available with all investigation types.
Note: This code may be used with hardcopy fingerprints if you agency is sending the hardcopy fingerprints for processing before the investigation request is sent to DCSA. Putting the I code in the FIPC Code field on the AUB in e-QIP whenever there is a completed fingerprint check (SAC, case type 92) in CVS/PIPS, will help automatically link the fingerprint with the higher-level case and bill accordingly.
 
R Advance Fingerprint results report is required by agency (See FIN 06-04 (PDF file) [777.17 KB]). This code will trigger an Advanced Fingerprint Report to be generated the day after completion of the criminal history check. That report is then immediately sent to the SOI in whatever method your agency currently receives investigative results. NOTE: If you use FIPC Code R and your fingerprints were scheduled as a SAC (case type 92 for the same SOI), the same fingerprint results will be sent to the SOI multiple times. Available with all investigation types.  
T FINCEN conducted. Only available for NACs submitted on the SF 86. X
