The Controlled Unclassified Information Executive Agent (CUI EA) develops resources for Executive branch departments and agencies that handle unclassified information that requires safeguarding or dissemination controls, pursuant to and consistent with applicable law, regulations, and government-wide policies.

CUI Coversheet

Agencies may use a coversheet to identify CUI, alert observers that CUI is present from a distance, and serve as a shield to protect CUI from inadvertent disclosure. If an agency chooses to use a coversheet, it must use CUI EA-approved coversheet Standard Form 901.

NOTE: Standard Form 901 replaces forms OF901, OF902 and OF903, which were rescinded on December 14, 2018. Agencies may continue to use Forms OF901, OF902, and OF903 until existing supplies have been depleted.

CUI Media Labels

SF 902 (CUI Media Label). SF 902 (approximate size 2.125 inches X 1.25 inches) is used to identify media that contains CUI (for example: hard drives). See CUI Notice 2019-01 for additional information. SF 902 is available for purchase from GSA. NSN 7540-01-679-3318
